Responsibilities
- Complete thorough assessments on referrals including intake and ongoing evaluation of needs
- Provide case management in the home, office or community. This may include linking to community resources, assisting with employment and housing needs; supervising visits; foster parent support; teaching parenting skills, etc
- Collaborate with other professionals and placing agencies-including DCS Family Case Managers, CASA volunteers, foster families, probation offices, and school personnel to coordinate treatment strategies
- Develop individualized treatment plans that incorporate goals aligned with family preservation and/or reunification
- Maintain accurate and supporting documentation within our online system while adhering to privacy standards
- Participate in weekly staffing with supervisor; attend staff meetings as required
Requirements
- Must be 21 years of age
- Must possess valid drivers license
- Must provide proof of valid automobile insurance
- Must pass all necessary background checks
Pay: From $42,000.00 per year
