- A valid driver’s license
- A degree from an accredited Occupational Therapy
- Ability to travel within Queens
- Ability to work independently with minimal supervision
- Strong organizational and communication skills
- Excellent interpersonal skills
- One year of home health experience (preferred but not required)
Responsibilities:
- Evaluate patients referred for home care services to determine the appropriateness of occupational therapy services.
- Establish a home health treatment plan with clear goals based on the initial assessment, and provide timely, effective treatment.
- Collaborate with agency personnel to ensure the agency meets the patient's needs.
- Explain the agency’s policies and services to patients and their families during the initial home care assessment.
- Complete and submit documentation accurately and promptly to the agency office.
- Maintain ongoing communication with agency personnel.
- Ensure customer satisfaction through professional, empathetic care.
