FACT Program Assistant

The Dutchess Reintegration Forensic ACT (Assertive Community Treatment) team is a multidisciplinary team including clinicians, nurses, prescriber(s), and case managers. The FACT teams primary focus is providing care to those individuals with serious mental illness, involved in the criminal legal system, whose needs have not been well met by more traditional service delivery systems. Services are delivered in the person’s home or community, available 24/7, and focus on helping individuals stay stable, avoid hospitalization, and build skills for independent living.

The Program Assistant provides critical administrative and operational support to the Forensic Assertive Community Treatment (FACT) team. This role helps ensure smooth daily functioning of the program by managing documentation, maintaining records, supporting communication, coordinating logistics, and assisting the multidisciplinary team in meeting service, compliance, and workflow needs. The Program Assistant contributes to efficient, recovery-focused services for individuals with severe and persistent mental illness.

Essential Duties & Responsibilities:

• Maintain organized, accurate client records, including intake materials, releases, assessments, service plans, and progress notes.

• Assist with data entry, reporting requirements, and tracking service contacts, appointments, and compliance metrics.

• Support billing processes by ensuring documentation is complete and submitted in a timely manner.

• Coordinate team schedules, meetings, orientations, and trainings; prepare agendas and take meeting minutes as needed.

• Manage program communication, including phone calls, messages, mail, and scheduling with clients and community partners.

• Assist with purchasing, inventory, and management of supplies, office equipment, and program resources.

• Help organize transportation schedules, client appointments, and outreach logistics.

• Provide general office support, including filing, scanning, printing, and maintaining organized electronic and physical records.

• Support onboarding processes for new staff by preparing materials and ensuring required documentation is completed.

• Uphold confidentiality, HIPAA compliance, and organizational policies at all times.

• Serve as a supportive, professional point of contact for clients, families, and collaborating agencies.

Qualifications:

• High school diploma or equivalent required; associate or bachelor's degree preferred.

• Prior administrative experience in human services, healthcare, or behavioral health settings preferred.

• Strong organizational, multitasking, and communication skills.

• Proficiency with Microsoft Office Suite, electronic health records, and database systems.

• Ability to work collaboratively in a fast-paced, multidisciplinary environment.

• Commitment to person-centered, trauma-informed, and recovery-oriented care.

Work Environment:

Primarily office-based with periodic interaction with clients and community providers. Requires strong attention to detail, professionalism, and the ability to manage competing priorities.

Successful completion of background check including Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check.
