Speech-Language Pathologist (SLP) - Richmond, VA
Job Summary:
Richmond Post Acute is seeking a compassionate, knowledgeable, and motivated Speech-Language Pathologist (SLP) to join our interdisciplinary rehabilitation team at Magnolia Post Acute. As a key member of our team, you will evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive-communication, voice, and swallowing disorders. Your expertise will play a vital role in helping residents regain independence, improve communication, and enhance their overall quality of life.
Essential Responsibilities:
- Evaluate residents to identify speech, language, cognitive, voice, and swallowing impairments
- Develop individualized treatment plans based on physician orders, clinical findings, and resident goals
- Provide evidence-based speech-language therapy services in accordance with state and federal regulations
- Assess and treat dysphagia, recommending appropriate diet modifications and safe swallowing strategies
- Collaborate with physicians, nursing staff, occupational therapists, physical therapists, dietitians, and other interdisciplinary team members to coordinate resident care
- Educate residents, families, and caregivers regarding treatment plans, progress, and home exercise programs
- Maintain accurate, timely, and compliant documentation of evaluations, treatments, progress notes, and discharge summaries
- Participate in care plan meetings, discharge planning, quality improvement initiatives, and facility education programs
- Ensure compliance with Medicare, Medicaid, HIPAA, and all applicable regulatory standards
- Maintain a safe, clean, and organized treatment environment
- Stay current on best practices through continuing education and professional development
Qualifications:
- Master's Degree in Speech-Language Pathology from an accredited program
- Current Speech-Language Pathologist license in the state of employment
- Certificate of Clinical Competence (CCC-SLP) preferred; Clinical Fellowship (CFY) candidates are encouraged to apply where permitted
- Current CPR certification (or willingness to obtain upon hire)
- Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred
- Strong clinical assessment and treatment skills
- Excellent communication, organizational, and interpersonal skills
- Ability to work independently while collaborating effectively within an interdisciplinary team
