Job Title: Education Continuity Coordinator
Location: Hybrid, Colorado & Maryland
Work Environment: Teen & Young Adult Outpatient Treatment Centers
Schedule: Monday to Friday | Some evenings required
Job Type: Full Time
Compensation: $25 - $30 per hour dependent of experience and licensure
About The Role
The Educational Continuity Coordinator (ECC) is responsible for ensuring uninterrupted access to educational rights and services for every adolescent and young adult enrolled in Sandstone Care's Partial Hospitalization Program (PHP). Operating within a dual-regulatory environment governed by both behavioral health licensing authorities and state education agencies, this role functions as the primary liaison between the treatment program and the adolescent's home school, Local Education Agency (LEA), post-secondary education organization and relevant educational officials across assigned states.
You will coordinate, facilitate, monitor, and document educational continuity-ensuring that every adolescent's academic progress, educational rights, and school relationships are actively maintained throughout treatment and that every student returns to their school of record with a clear, documented, and actionable transition plan.
Key Responsibilities:
- Complete comprehensive educational screenings of incoming clients
- Interpret Individualized Education Plans (IEP) or Section 504 Plans to plan for, facilitate and measure continued learning
- Provide academic & vocational services, including delivery of assessments (when appropriate), building goals and measurable objectives, and coordinate academic needs with outside school or BOCES, as well as vocational and recreational services with families
- Timely documentation of services
- Assists and encourages clients in the completion of homework assignments
- Must consistently demonstrate the ability to establish and maintain good public relations with schools, families, referral agencies, and other community resources
- Must have the ability to express ideas clearly, concisely, convincingly, and effectively when addressing an audience
- To consistently exercise good judgment and adequate emotional stability in evaluating situations and making decisions despite the pressures of deadlines or the occurrence of unexpected stressful events
- May be asked to participate in IEP meetings
- Coordinates and supports the daily structured study period
- Collaborating with the Site Director, monitors anticipated length of stay for all active clients
- Maintains a running school communication log for every active client
- Completes written Academic Transition Plan for every client prior to discharge
- Participates in monthly quality assurance reviews
Qualifications:
Education
- Bachelor's degree required, Bachelors or Master's Degree in Education or in a Behavioral Health Science (clinical social work, clinical psychology, marriage and family therapy, or a related field) is preferred.
Training/Experience
- Certified or is eligible for a teaching license in the state in which you work
- Working knowledge of Individuals with Disabilities Education Act (IDEA)
- Experience providing mentoring, academic, vocational, and life skills support to adolescents
- Ideal candidate has a minimum of 3 years of experience, including experience with clients from the age of 13 to 18
- Strong written and verbal communication skills.
