That's why we're seeking a compassionate Licensed Clinical Social Worker (LCSW) to provide counseling, emotional support, patient advocacy, and resource coordination for individuals and families facing some of life's most challenging moments.

- Provide therapeutic counseling, crisis intervention, and emotional support to patients and families.
- Conduct psychosocial assessments and develop individualized support plans.
- Participate in distress screening follow-up, survivorship care, and advance care planning discussions.
- Connect patients with financial, community, and supportive resources.
- Serve as a patient advocate and help navigate healthcare and community systems.
- Facilitate support groups and collaborate with community organizations.
- Partner with providers and clinical staff to address psychosocial barriers to care.
- Maintain timely and accurate documentation in the electronic medical record.
- Master's degree in Social Work (MSW) from a CSWE-accredited program.
- Current Virginia Licensed Clinical Social Worker (LCSW) license in good standing.
- Minimum of three (3) years of clinical social work experience in a healthcare setting.
- Experience in oncology, palliative care, hospice, behavioral health, or other medically complex patient populations preferred.
- Experience facilitating support groups, patient education programs, or community-based support services.
