Solace Hospice and Palliative Care is a CMS-certified, Joint Commission–accredited hospice serving patients and families in the St. Charles, IL area. We're seeking a compassionate Nurse Practitioner to join our interdisciplinary care team.
About the Role The NP delivers age- and specialty-appropriate medical care to hospice patients under the supervision of the Medical Director, and may serve as Hospice Attending Physician when selected by the patient at admission. This role blends direct clinical care, Face-to-Face eligibility recertification, and close collaboration with our interdisciplinary team.
Responsibilities
- Conduct Face-to-Face (F2F) encounters for hospice recertification per CMS requirements (42 CFR §418.22)
- Confirm and document continued hospice eligibility based on disease progression
- Collaborate with the Medical Director and IDT on plan-of-care development and updates
- Provide direct palliative care and symptom/pain management
- Educate patients, families, and IDT members on disease process and end-of-life care
- Maintain strong relationships with referring physicians, facilities, and payers
- Participate in on-call rotation
Required Qualifications
- Active, unrestricted RN license in the State of Illinois
- Current Illinois APRN/Nurse Practitioner certification
- Minimum 1 year of Home Health and/or Hospice clinical experience (required)
- Minimum 2 years of clinical NP experience
- Valid Illinois driver's license and reliable transportation
- Working knowledge of Medicare Hospice Conditions of Participation (42 CFR Part 418)
- Ability to work independently with strong clinical judgment
Physical Requirements Ability to sit, stand, walk, bend, kneel, and lift up to 50 lbs; full body mobility; vision, color perception, and hearing sufficient for clinical assessment.
