Job Summary
The LTC Clinical Coordinator job duties are considerable, and vary depending on size, level of care, managerial structure, and focus. Generally, this position acts as the long-term care expert in regards to regulatory compliance, risk management, survey preparedness, and long-term care policies, procedures and protocols for Jones County Rest Home.
Minimum Qualifications
- Current unrestricted Registered Nurse license in the state of Mississippi.
- Prefer 5 years’ experience in long term care environment, or other related field
Professional Responsibilities
- Provide a positive and professional representation of the organization.
- Promote a culture of safety through reporting, documentation, prevention, and infection control.
- Maintain competency and knowledge of current standards of practice, trends, and developments in related scope of job role or practice.
- Maintain compliance with organizational and regulatory policies and practices.
- Demonstrate excellent customer service through attitude and actions, consistent with the standards contained in the Vision, Mission, and Values of the organization.
Essential Duties & Responsibilities
- Participate in ongoing education to maintain current professional standards and licensure
- Ensure the facility operates in compliance with all local, state, and federal regulations
- Be present for and assist with survey process as well as plan of correction
- Conduct mock surveys quarterly or as needed to ensure survey preparation and readiness on an ongoing basis
- Ensure that all policies, procedures, and protocols are in place to adhere to regulatory compliance
- Assist with developing and/or conducting staff, resident, family, and visitor education as needs are identified. Including return demonstration with staff to ensure competence
- Actively participate in continuous performance improvement initiatives, committee meetings, and auditing activities at least monthly
- Provide oversight of LTC electronic medical record including education of updates and guidance on staff competence of system
- Oversee the timely completion of ongoing education assignments for facility staff and ensure compliance is met
- Oversight of LTC Medical Director Meeting including scheduling, invites, minutes, etc
- Actively assist with coordination of information for consultants and audits. Provide oversight, consultation and guidance with CASPER reports, facility QMs, 5 Star reports, VBP, etc.
- Auditing of EMR to ensure high risk areas are identified and DON is informed of any concerns. Provide education and training as needed in problematic areas
- Provide assistance in the investigation of resident, family and employee concerns, such as but not limited to, reportable events, allegations of abuse, and Employee Occurrence Reports. Assist in identifying root cause and provide recommendations for corrective action plans.
- Assist in completion of the facility assessment per regulatory guidance
- Work with administration to upgrade and improve resident care, develop nursing organization, and set standards for qualifications of nursing personnel
- Assist with prior authorizations, pharmacy charges and reconciliation
- Communicate effectively with all departments to ensure quality improvement and policy compliance standards are met
- Promote effective working relationships with all departments
- Monitor staffing ratios to ensure state requirements and resident care needs are met
- Perform other job duties and tasks as assigned
Physical Requirements
- Ability to stand and/or walk for extended periods.
- Frequent bending, reaching, squatting, and kneeling.
- Lift, carry, push, and pull up to 50 lbs independently; >50 lbs with assistance/devices.
- Assist with resident positioning and transfers using proper body mechanics.
- Manual dexterity for clinical tasks and computer use; adequate vision, hearing, and speech.
