Duties

• Prepare, dispense, and administer medications in accordance with physician orders, including injections, IV infusions, and other treatments within the medication room.
• Monitor patients’ vital signs, assess their condition, and document responses accurately using electronic health record (EHR) systems such as Epic or Cerner.
• Ensure strict adherence to HIPAA regulations and maintain confidentiality of all patient information.
• Assist with patient assessments, including history taking and physical examinations related to medication therapy and treatment plans.
• Manage medication inventory, verify proper storage conditions, and document all medication transactions meticulously.
• Collaborate with interdisciplinary teams to coordinate care plans for individuals with complex needs such as dementia, Alzheimer's disease, or developmental disabilities.
• Provide education on medication administration, side effects, and safety protocols to patients and their families.

Qualifications

• Valid Licensed Practical Nurse (LPN) license with current state certification required.
• Proven experience working in critical care settings such as ICU or emergency medicine preferred; hospital or long-term care experience highly valued.
• Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for documentation and patient records management.
• Knowledge of medical terminology, anatomy, physiology, and pharmacology related to medication administration.
• Experience with IV infusion therapy, catheterization, venipuncture, aseptic technique, and sterile processing techniques.
• Strong assessment skills including vital signs measurement and patient evaluation; capable of triage in urgent situations.
• Ability to work effectively with diverse populations including seniors, individuals with disabilities or memory care needs, and those requiring acute pain management.
• Excellent communication skills for patient education and interdisciplinary collaboration; commitment to confidentiality under HIPAA standards.
