Supportive Concepts for Families is seeking compassionate Registered Nurses to serve as Community Health Nurses, providing clinical oversight and care management for adults with intellectual and developmental disabilities living in community homes throughout Pennsylvania.
This is a field-based nursing role where you'll work independently while collaborating with physicians, specialists, Direct Support Professionals, and interdisciplinary teams to ensure exceptional care for individuals with complex medical needs.

What You'll Do
Manage a caseload of individuals with complex medical needs.
Coordinate care with physicians, hospitals, specialists, pharmacies, therapists, and families.
Complete nursing assessments and individualized care plans.
Monitor chronic conditions, dysphagia, specialized diets, seizure disorders, diabetes, wound care, fall risks, and other high-acuity medical concerns.
Participate in interdisciplinary team meetings and advocate for quality healthcare.
Train and support Direct Support Professionals on medical protocols and best practices.
Review physician orders, laboratory results, medication changes, and clinical documentation.
Promote regulatory compliance, infection prevention, and quality improvement initiatives.
Help reduce avoidable emergency department visits and hospitalizations.
Qualifications
Active Pennsylvania Registered Nurse (RN) license in good standing.
Valid driver's license and reliable transportation.
Strong assessment, critical thinking, and clinical decision-making skills.
Excellent communication and collaboration abilities.
Experience in IDD, long-term care, home health, case management, behavioral health, or community nursing is preferred but not required.
