POSITION DETAILS
Schedule: 1–2 days per month (flexible scheduling)
RESPONSIBILITIES
- Provide medical nutrition therapy and individualized nutrition counseling
- Educate patients on nutrition related to diabetes, hypertension, cardiovascular health, weight management, metabolic health, and preventive nutrition
- Collaborate with nurse practitioners and clinical staff to support patient care plans
- Document visits appropriately in the electronic medical record Provide culturally sensitive, patient-centered counseling

- Active New York State licensure/certification (or eligibility)
- Comfortable working with children, adults, and geriatric patients
- Strong patient education and communication skills
- Experience in outpatient, primary care, or community health settings preferred Bilingual English/Spanish a plus
