In this role, you will provide individualized case management and vocational support to adults with intellectual and developmental disabilities, helping them build employment skills, achieve personal goals, and work toward greater independence.
As a Case Manager, you'll collaborate with participants, families, funding agencies, employers, and community partners to develop person-centered vocational plans while providing the guidance and support needed for long-term success.

Essential ResponsibilitiesCase Management & Individualized Planning- Develop, implement, and monitor Individualized Service Plans (ISPs), Rehabilitation Plans, and other person-centered service plans with measurable vocational goals.
- Provide ongoing case management, supervision, coaching, and support to assigned participants.
- Monitor participant progress and adjust vocational plans as needed to promote successful outcomes.
- Maintain accurate case notes, documentation, reports, and participant records in accordance with agency and funding source requirements.
- Complete monthly, quarterly, and annual reports as required.
- Provide individualized job readiness, career exploration, workplace, and life skills training.
- Assist participants in developing vocational, community employment, and self-employment opportunities based on their interests, abilities, and goals.
- Provide direct support and supervision within vocational programs, community worksites, and employment settings.
- Implement workplace accommodations, job modifications, and individualized supports to promote successful employment outcomes.
- Provide crisis intervention, counseling, and problem-solving assistance when necessary.
- Serve as a positive role model and advocate while promoting independence, professionalism, and community inclusion.
- Conduct and coordinate participant intakes, assessments, evaluations, and eligibility determinations.
- Review participant progress and determine continued eligibility for vocational services, coordinating transfers or program transitions when appropriate.
- Participate in production floor time studies and quality assurance activities.
- Assist with program expansion efforts, including identifying community employment opportunities and supporting grant-related initiatives when assigned.
- Maintain compliance with agency policies and regulatory requirements established by CARF, DDD, DVRS, DCF, DOE, DOL, and other funding or oversight agencies.
- Build positive working relationships with participants, families, employers, funding sources, residential providers, and community partners.
- Participate in audits, inspections, licensing reviews, and quality improvement initiatives.
- Complete Unusual Incident Reports (UIRs), General Event Reports (GERs), and other required documentation accurately and within required timelines.

Requirements
- Bachelor's Degree in Human Services, Special Education, Psychology, Social Work, or a related field required.
- Minimum of one (1) year of supervisory experience working with individuals with intellectual or developmental disabilities.
- Valid New Jersey driver's license in good standing with no more than five (5) points.
- Strong leadership, communication, organizational, and case management skills.
- Ability to effectively supervise participants while coordinating multiple priorities.
- Ability to work independently and collaboratively in both office and community-based settings.
- Must successfully complete all required background screenings, including Criminal History, Central Registry, CARI, and applicable drug testing requirements.
- Must complete required Division of Developmental Disabilities (DDD) pre-service trainings and maintain CPR/First Aid certifications as required.
