Qualifications:
- Current Louisiana Licensed Practical Nurse (LPN) license required
- Minimum 1 year clinical experience as an LPN
- Current CPR Certification
- Compassionate and great time management skills
- Reliable transportation to conduct patient visits at their place of residence or facility
Job Duties:
- Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
- Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
- Document all services provided.
- Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
- Report changes in patient condition and needs to RN
