The Center for Sleep Medicine is a forward-thinking, 100% employee-owned medical practice accredited by the AASM. We are actively searching for an experienced, enthusiastic, and results-driven Patient Care Coordinator (PCC) to join our team based in the Libertyville, IL area.
This is a hybrid role, with two days spent in the office and three days in the field (typical hours are M-F / 8:30 AM - 5:00 PM).
The PCC serves as the liaison to referring physicians, combining provider engagement, patient care coordination, and referral development. You will build collaborative relationships with referral sources while supporting growth across our accredited sleep centers. The PCC also assists with the development of marketing strategies and collateral while providing ongoing account management through our CRM platform.
Responsibilities
- Articulate The Center for Sleep Medicine's value proposition and differentiators.
- Develop, maintain, and grow collaborative relationships with internal and external stakeholders.
- Build a robust pipeline of new business through strategic prospecting, partner referrals, and collaboration with internal teams.
- Stay ahead of market trends with referral sources while proactively identifying new opportunities.
- Be proficient using CRM and Microsoft Office to drive strong territory and account management.
- Utilize business development best practices to drive measurable results.
- Achieve quarterly territory growth and referral development goals.

Qualifications
- CRM experience preferred.
- Excellent written and oral communication skills.
- Candidates must have the ability to plan and carry out responsibilities with minimal direction, act with intellectual honesty and integrity, and be motivated, persistent, and detail-oriented.
Education:
- Bachelor's degree in Business, Marketing, Healthcare Administration, Life Sciences, or a related field preferred.
Required Experience:
- Two or more years of experience in healthcare sales, physician relations, business development, provider engagement, or a related healthcare environment preferred.
- Experience developing relationships with physicians, advanced practice providers, referral sources, or healthcare organizations.
- Proficiency with CRM platforms, Microsoft Office, and other business applications preferred.
- Knowledge of sleep medicine, PAP therapy, DME, RPM/RTM, or related healthcare services is desirable but not required.
- Equivalent combinations of education, training, and relevant healthcare experience may be considered.

Relationship Skills: Build rapport, develop professional relationships, and maintain provider confidence.
Business Development Skills: Prospect new opportunities, communicate value, overcome objections, and achieve territory growth objectives.
Professional Skills: Time management, problem-solving, self-motivated, organized, and able to travel locally.
Licenses/Certificates:
Must maintain a valid driver's license.
Salary Range
$50,000.00 - $60,000.00 per year
- uncapped commission opportunity based on quarterly metrics.

This pay range reflects the Company's current estimate for this position at the time of posting.
