About the Position:
Provide goal-directed in-home counseling/casework interventions to persons sixty years of age or older to improve quality of life and prevent nursing home placements. Seek out and provide one on one contact with older adults or their caregivers to encourage use of existing services and benefits.
Develop a supportive relationship with clients and assist children and families in addressing needs and meeting goals with the overall objective of achieving solutions that promote stability and permanency.
A Typical Day as a Community Outreach Case Manager and Senior In-Home Counselor and Outreach Worker:
- Complete initial assessments and develop treatment plans based on client's personal goals and presenting needs.
- Provide counseling services in clients' homes in accordance with assessment information and treatment plan with overall purpose of addressing mental health needs and preventing unnecessary nursing home placements.
- Conduct in-home visits and inform persons of benefits and services available to them.
- Assist older persons in gaining access to needed services such as making referrals for them.
- Document all services in accordance with agency, state, and federal requirements.
- Conduct collateral contacts with individuals significant to assessed problem situation.
- Execute referrals to those community resources providing services which address relevant identified problem circumstances, with client's permission.
- Support children, teens, adults, and families that are experiencing crisis
- Offer families a variety of tools to help in their success by using a trauma-responsive and family-centered approach
- Collaborate with clients to develop strength's based goals
- Engage with families and clients by visiting them in a variety of settings to support their goals
Location:
2610 W. Richwoods Blvd. Peoria, IL with the willingness to travel and spend several hours in the field visiting clients' homes and communities. This office provides services to the following counties: Peoria, Woodford, Tazewell, Mason, and Fulton
Work Hours:
Schedule is flexible and non-traditional; based around the availability of youth and families served
Does the Following Apply to You?
- Bachelor's Degree
- 1-3 years working with older persons (preferred)
Additional Qualifications:
- Completion of DCFS criminal background check and fingerprinting
- Must be at least 21 years of age
- Obtain an insured vehicle
- At least three years of driving experience with a valid driver's license
