About Us:
The Inpatient Pharmacy Department at Valley Children’s utilizes evidence-based therapeutic guidelines to provide exceptional pharmaceutical care to our patients. Our highly trained pharmacists and pharmacy technicians work side-by-side with providers and other interprofessional teams to ensure optimal patient outcomes, while utilizing state-of-the-art technology to maximize patient safety.
Inpatient Pharmacy provides clinical interdisciplinary pharmacy services in all inpatient areas including Rehab, PICU, NICU, Acute Care, and Oncology. We have numerous pharmacy advance practice protocols including antibiotic monitoring, anticoagulation, IV to oral conversion, and renal dosing. Other clinically supported services include the Emergency Department, Palliative Care, Investigational Drug oversight, and parenteral nutrition (including production).
Our team members also support the following specialty clinics: Oncology, Hematology, Rheumatology, Pulmonology/CF, Endocrinology, Neurology, Cardiology and our complex primary care center. Many pharmacists hold collaborative practice agreements to optimize medication therapy management and collaborate to enhance patient care.
Job Summary:
This position is accountable for providing safe, effective, and evidence-based pharmaceutical care within ambulatory specialty clinics at Valley Children's Healthcare. Serves as a pharmacology consultant, a patient advocate, and medication-use resource for patients, families, providers, nurses, and other members of the healthcare team. Facilitates optimal medication outcomes through comprehensive medication management, education, care coordination, monitoring of therapeutic outcomes, and development of individualized treatment plans. Works collaboratively with the interdisciplinary team to improve quality, safety, access, and cost-effective mediation use across ambulatory care populations. Practice responsibilities may include disease state management, medication monitoring, protocol-driven care, medication access and continuity of care support, and activities performed under collaborative practice agreements as applicable.Qualifications:
Education
- Bachelors Degree Pharmacy (required)
- PharmD (preferred)
- Program ASHP Accredited Residency Program (preferred)
Licenses and Certifications
- RPH - Registered Pharmacist - CA-SBP - California State Board of Pharmacy California State Board of Pharmacy (required)
- BLS - Basic Life Support - AHA - American Heart Association within 30 days of hire or transfer into position (required)
- Industry Certification - Other Board of Pharmacy Specialties Certification (preferred)
Work Experience
- Minimum one (1) year Experience in pediatrics and/or home care preferred. (preferred)
- Residency (preferred)
Skills and Abilities
- Application of pharmacology and knowledge of drug distribution.
