Valley Health Partners, Allentown, PA
Job Summary:
Provides a full range of mental health outpatient services including initial assessment, treatment planning, individual and group psychotherapy, suicide or other risk assessment, as well as crisis intervention, case management, and discharge planning.
Qualifications:
Education:
- Master’s Degree in Social Work, Counseling, or related mental health field.
Experience:
- 2 years of clinical experience in a behavioral health setting.
License:
- Licensed Clinical Social Worker (LCSW) in PA, or Licensed Professional Counselor (LPC) in PA, upon hire.
Training:
- CPR / BLS upon hire.
Special Skills:
- Demonstrated skill in clinical assessment and therapeutic techniques.
- Demonstrated knowledge of behavioral health field and resources.
Essential Functions:
- Provides culturally sensitive individual, family, and group psychotherapy in support of the patient's treatment plan monitoring progress toward goals and adjusting treatment approaches, as indicated.
- Provides psychosocial assessments including clinical impressions and treatment plan, in collaboration with the patient.
- Monitors for and addresses suicide/high risk as indicated by professional and departmental standards.
- Provides patient and family education based on treatment needs.
- Performs case management as required by patient need.
- Provides timely discharge planning with a grasp of community supports; educates patient/facilitates referrals based on treatment needs.
- Documents services in a timely fashion and in adherence with professional, program, departmental, network, regulatory, and third-party payor standards.
- Takes on other duties as assigned.
