TITLE: Pharmacist, Centralized Refill Authorization
JOB OVERVIEW: The Pharmacist, Centralized Refill Authorization is responsible for providing refill authorization services to Primary Care providers in the VMC Clinic Network. The pharmacist acts as a liaison between patients, providers, and pharmacies to ensure that refill requests are reviewed and processed promptly and accurately, maintaining high levels of customer service and patient satisfaction.
DEPARTMENT: Pharmacy Services
WORK HOURS: Variable to accommodate Pharmacy needs.
REPORTS TO: Clinical Ambulatory Pharmacy Operations Manager
PREREQUISITES:
Graduate of an accredited College of Pharmacy required; PharmD degree strongly preferred.
Licensed as a Registered Pharmacist in the State of Washington with license in good standing. required.
Minimum of 2-3 years Pharmacist experience in a retail pharmacy setting, preferred.
Prior experience using Epic preferred.
QUALIFICATIONS:
Professional and effective verbal and written communication skills.
Strong customer service orientation and placing priority on serving the needs of patients, their families, and health care providers.
Demonstrate ability to exercise independent judgment and discretion.
Demonstrate ability to multitask, set priorities, and work accurately and independently.
Demonstrated ability to develop and maintain positive interpersonal relationships with fellow employees.
Demonstrated ability to work effectively in a team environment.
Demonstrated knowledge and problem-solving skills related to all aspects of pharmacotherapy, disease-state management, and drug-drug interactions.

Requires the maintaining of a knowledge base of current medications, drug therapy guidelines, drug therapeutic interchange, and updates to FDA-approved indications.
Knowledge of federal and state laws, regulations and standards governing the practice of pharmacy.
Knowledge of Third-Party Insurances and prior authorization process.
Requires competency in all applicable collaborative drug therapy agreements.
Ability to precept students and residents.

B. Essential Responsibilities and Competencies:
Adheres to department of pharmacy and organizational policies and procedures in all areas of practice.
Responsible for providing high-quality pharmaceutical care to patients Valley Medical Center's Clinic Network. This includes ensuring accuracy and appropriateness of refill authorizations, review of generic substitutions and therapeutic interchanges, and conducting accurate drug utilization reviews (DUR).
Accurately maintain patient's profile for medication intolerance and/or allergies.
Respond to patient inquiries both verbally and in writing regarding refill status, dosage instructions, and general medication related questions within the scope of the role.
Communicate patient needs and concerns to providers and clinic staff to ensure timely resolution of refill related issues.
Responsible for promoting safe and effective use of medications to facilitate an appropriate therapeutic outcome for the patient.
Collaborates with providers and other health care professionals to individualize appropriate drug therapies based on patient parameters and interpretation of current literature, safety, efficacy, and cost.
Participate in medication therapy management including monitoring and modifying drug therapy per established protocol.
Participate in chronic disease state monitoring and utilize registries and/or other databases as available to enhance therapeutic outcomes through disease/medication education, adherence monitoring, and drug therapy optimization.
Responsible for appropriately supervising the work completed by support staff.
Comply with HIPAA and all other applicable federal and state laws and regulations.
Identify patient barriers to medication adherence considering patient characteristics (including language, cultural/religious practices, physical, cognitive, emotional, pain/discomfort, socioeconomic status, etc.).
Responsible for documenting interventions, drug information questions, drug utilization evaluation activities, and adverse drug reactions.
Participates in the development and documentation of quality assurance indicators such as drug use evaluation.
Maintain clinical competency through successful completion of the self-study modules and providing clinical services in practice.
Oversee pharmacy residents, technicians, interns, and students, as well as training new pharmacists.
Participates in projects as determined by the strategic goals and initiatives of the department and organization.
Conduct periodic reviews of prescribing practices and design targeted prescriber education to promote compliance with clinical practice guidelines.
Actively participate in committees responsible for medication use, quality improvement, performance improvement, and other aspects of patient care within the health system.
Maintain competency in all applicable CDTAs and complete all department and organizational competency assessments as assigned.
Provide professional and courteous support to patients and clinic staff.
Completes additional responsibilities/projects as assigned by Supervisor/Manager/Director.
