The Director of Product Management for Care Coordination is the single accountable product owner for Veradigm's care coordination and chronic disease management products and capabilities. This portfolio spans both the Provider and Payer sides of the business, where care coordination is delivered primarily through chronic disease management, care management, and population health workflows. Reporting to the Vice President of Product Management, this leader sets and executes the holistic care coordination product strategy, from short-term release planning through multi-year vision, ensuring every product decision is grounded in deep customer insight, healthcare market dynamics, and Veradigm's business objectives.
This is a largely greenfield area, so a core mandate of this role is to build the care coordination product from the ground up, defining what we offer and designing it to be scalable and repeatable from day one. You will set the priorities, decide where to start, and drive the highest-value opportunities from discovery through delivery.
This role starts as an individual contributor. You will own care coordination product directly until it is off the ground, with a team to lead as the product matures. From day one you will drive cross-functional alignment across Engineering, Design, Sales, Marketing, and Client Services. Because care coordination touches both provider and payer customers, success depends on working effectively across organizational and product boundaries. In some cases you will build new products from scratch with a dedicated engineering team. In other cases you will define requirements and influence roadmaps owned by other product teams across Veradigm. Either way, you are accountable for outcomes, not just output.
We operate under an empowered product team model rooted in SVPG principles. Product managers own the problems (not just the features), teams are cross-functional and empowered to find the best solutions, and success is measured by business and customer outcomes. This role carries significant ownership and requires a leader who thrives in that kind of environment.
Essential Functions & Key Responsibilities
Product Strategy, Vision & Productization
- Own the end-to-end care coordination product strategy across the Provider and Payer segments, defining short-term (current to next release) and mid-to-long-term roadmaps that align with Veradigm's corporate direction and the evolving value-based care market.
- Assess the current care coordination and care management portfolio and identify the highest-value opportunities to productize, building a clear framework for what should be a product, what should stay a service, and what should be retired.
- Translate market trends, regulatory changes (e.g., value-based care programs, CMS interoperability rules, quality measurement under HEDIS and Stars, chronic care management billing), and customer needs into a coherent product vision that differentiates Veradigm in a competitive landscape.
- Partner with the VP of Product Management and executive leadership to align care coordination strategy to overall company goals, communicating trade-offs and opportunities with clarity and confidence.
- Identify white space and adjacency opportunities across the care coordination domain (for example, risk stratification, care gap closure, transitions of care, remote monitoring, and patient engagement), and build business cases for net-new investment, partnerships, or M&A consideration.
Product Discovery & Customer Insight
- Lead structured discovery programs, including customer interviews, site visits, advisory councils, and quantitative research, to surface unmet needs and validate hypotheses before committing to build.
- Apply SVPG principles: assess value, usability, feasibility, and viability before committing engineering resources.
- Develop and maintain a deep understanding of care coordination customers on both sides of the market, including provider organizations (independent practices, health systems, and care management teams) and payers (health plan care management, population health, and quality functions), and their distinct workflows, pain points, and success metrics.
- Synthesize competitive intelligence and market data into actionable insights that continuously sharpen the product direction.
Product Development & Delivery
- Define and communicate clear, outcome-driven product requirements and prioritized backlogs that empower engineering and design teams to deliver effectively.
- Partner closely with Engineering and Design as a true product trio, empowering the team to solve problems rather than handing down specifications.
- Where productization requires capabilities owned by other teams, define clear requirements and build strong cross-team relationships to influence their roadmaps.
- Own feature prioritization and trade-off decisions, maintaining a backlog that balances innovation, technical health, compliance requirements, and customer-reported issues.
- Drive a high-quality product experience, maintaining relentless attention to usability, reliability, performance, security, and scalability across the care coordination portfolio.
Go-to-Market & Commercial Enablement
- Serve as the chief product evangelist for care coordination, translating complex capability into compelling narratives for customers, prospects, sales teams, and industry audiences across both provider and payer markets.
- Collaborate with Marketing to develop positioning, messaging, and content that reflects the product's unique value and resonates across provider and payer buyer personas.
- Own field readiness for new releases, ensuring Sales, Customer Success, and Support are equipped with the training, tools, and knowledge to successfully position and support care coordination products.
- Support key deals, strategic accounts, and RFP responses by providing expert-level product input and participating in customer-facing conversations.
Team Leadership & Organizational Development
- Lead, mentor, and grow a team of product managers, establishing clear goals, performance expectations, and career development pathways.
- Foster a customer-centric, data-informed, and outcome-oriented product culture. Ensure PMs are spending time on discovery, talking to customers, and making evidence-based decisions.
- Model the behaviors expected of product leaders at Veradigm: intellectual honesty, customer obsession, and a bias toward learning over being right.
- Recruit top product management talent and build an inclusive team that reflects the diversity of Veradigm's clients and communities.
- Regularly communicate product progress, wins, and challenges to stakeholders across the organization, creating alignment and shared accountability.
Compliance & Regulatory Awareness
- Ensure all care coordination products meet applicable regulatory and compliance standards, including HIPAA, CMS rules relevant to care management and chronic care management, interoperability and information blocking requirements, quality reporting, payer requirements, and Veradigm's internal quality standards.
- Proactively monitor the regulatory environment, including value-based care policy and quality measure changes, and incorporate upcoming changes into product planning to reduce client risk and maintain market credibility.
