Primary Care Nurse Practitioner or Physician (MD/DO) – Dutchess County, NY
Part-Time | Three Days Per Week | Wappingers Falls, Beacon & Poughkeepsie
Visiting Docs is looking for an experienced Nurse Practitioner or Physician (MD/DO) to provide onsite primary care services at assisted living communities located in Wappingers Falls, Beacon, and Poughkeepsie, New York.
This position offers a predictable three-day weekday schedule and the opportunity to provide high-quality, relationship-focused care to older adults while traveling between a small group of established facilities.
Position Summary
- Part-time position
- Three weekdays per week
- Daytime hours only
- No nights, weekends, or overnight call
- Travel between assisted living communities in Wappingers Falls, Beacon, and Poughkeepsie
Responsibilities
- Deliver comprehensive primary care services to assisted living residents
- Conduct new patient evaluations, annual wellness visits, and routine follow-up appointments
- Assess, diagnose, and manage acute and chronic medical conditions
- Prescribe medications and develop individualized treatment plans
- Coordinate care with facility nursing staff, specialists, caregivers, and family members
- Complete timely and accurate documentation in the electronic health record
Qualifications
- Active New York State Nurse Practitioner or Physician (MD/DO) license
- Current DEA registration
- Active Medicare and Medicaid provider enrollment
- Ability to practice independently
- Nurse Practitioners must satisfy New York State's 3,600-hour independent practice requirement
- Prior experience in primary care
Preferred Background
- Geriatrics or internal medicine
- Assisted living, long-term care, or post-acute care experience
- Strong communication, documentation, and organizational skills

Candidates must already be enrolled as Medicare and Medicaid providers. Due to the extended credentialing process, we are unable to consider applicants who have not completed both enrollments.
