Position Summary:
Clinical Case Managers provide comprehensive psychological and behavioral case management services within a continuum of high to low intensity services designed to improve or maintain each person we serve's abilities to function in the community. Assesses, monitors, and follows up with person we serve's ability to meet basic needs in moving toward recovery. Responsible for all clinical documentation of consumer interaction.
As a Clinical Case Manager you will be providing transportation the people we serve in your vehicle (additional cost for insurance reimbursed).
Work Environment:
- Field-based role with regular visits to jails, courts, hospitals, and client residences.
- Transportation required (mileage reimbursed with business-use insurance).
- Trauma-informed, recovery-oriented organization.
Compensation & Benefits:
- CM I - $30.12/hr. - $62,650 annually (Bachelors Degree)
- CMII -$31.25/hr. - $65,000 annually (Master's Degree)
- Language Differential - $1hr - $2.50/hr for bilingual proficiency in Spanish & ASL

The posted range represents the full hiring range. Offers will not exceed this range, the majority of candidates are placed near the midpoint based on experience and qualifications.
Job Duties & Functions:
- Goal: Divert individuals likely incompetent to proceed from the criminal justice system into care coordination.
- Eligibility: Must have a qualifying misdemeanor/felony, past incompetency finding, current screening indicating likely incompetency, and DA consent.
- Referrals: Accepted from Diversion List, defense/DA, jail behavioral health, or court.
- Voluntary Participation: Individuals must agree to engage in the program.
- Process: Referrals go to the BRIDGES Wraparound Care Team. Within 30 days, BRIDGES develops a care plan with ROI for WellPower.
- Outcome: Upon successful completion, related charges are dismissed/sealed by the DA.
- HITT and CTT Overview:
- People we serve meet with a multidisciplinary team as they work towards wellbeing. Team = case managers, nurse practitioners and/or psychiatrists, nurses, and others as needed to support an individual's needs.
- Case managers are the main point of contact for the people on their caseloads
- At this level of care, case managers are primarily community based so that they can help people we serve attend to their needs in their natural environments. Case managers transport the people on their caseloads to various locations/agencies as part of linkage to community resources.
- Must have valid Colorado Driver's license, safe and dependable vehicle, and a recent clean driving record. Ability to transport the people we serve in your vehicle. Ability to obtain and carry Business Use coverage on your auto policy (additional cost is reimbursed).
- Basic Needs & Benefits: Assist clients with accessing food, shelter, clothing, personal safety, healthcare, and public benefits; coordinate applications for income and housing; arrange protective payeeships as needed. Identify persons for whom establishment of a protective payee-ship is necessary and arrange this assistance.
- Legal System Support: Attend court with clients (including Competency Docket & Outreach Court); coordinate and provide transportation; maintain jail clearance and Securus access; track referrals.
- Outreach & Engagement: Conduct weekly outreach in jails, shelters, hospitals, and the community; meet clients at jail releases and inpatient facilities; educate eligible individuals on available services.
- Skill Development: Support daily living skills (budgeting, hygiene, medication use, etc.); assist with employment referrals and housing placement; encourage meaningful social and recreational activity.
- Collaboration: Attend multidisciplinary meetings; coordinate with legal, clinical, and housing partners; provide support and education to families, landlords, and other community stakeholders.
- Population Served:
Justice-involved adults with behavioral health needs, including individuals found incompetent to proceed or eligible for diversion services.
Skills
- Knowledge or ability to learn and practice trauma informed principles and practices.
- Knowledge of methods of psychotherapy.
- Knowledge of community resources.
- Ability to assess crisis situations and intervene appropriately.
- Familiarity with the DSM-V and diagnostic techniques.
- Effective written and verbal communication skills.
- Ability to work flexible hours which may be required.
- Ability to give, receive and analyze information, formulate work plans, maintain confidentiality, prepare written materials and articulate goals and action plans.
- Must appreciate people from all walks of life and be able to communicate effectively with them.
- Must have a clear understanding of HIPAA, confidentiality, personal boundaries and be self-assured in a variety of situations.
- Have the ability to navigate crisis situations, de-escalate and remain calm.
- Someone who is energetic, with a positive and creative attitude is necessary.
- Performs other duties as assigned.
Requirements & Qualifications:
EDUCATION:
- Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or other related human services degree.
EXPERIENCE:
- One year experience in mental health or human services preferred.
- Must have valid Colorado Driver's license and a recent clean driving record. Ability to transport the people we serve in your vehicle.

Work Location: 4455 E 12th Ave. Denver, CO 80220
Shift/Hours: This is a Monday-Friday opportunity observing standard business hours.
Physical Requirements: Ability to sit, stand, bend and reach, drive a vehicle, operate standard office equipment such as phone, computer, facsimile, copier/printer, and other business machines. Occasional lifting up to 40 lbs.
