West County Health Centers (WCHC) provides comprehensive and quality health care services to the communities of Western Sonoma County. Position is based out of the Gravenstein Community Health Center in Sebastopol, CA 95472, with occasional travel to our Russian River Health Center in Guerneville, CA 95446 required.
Current schedule need is 16-18 hours/week on-site on Wednesdays and Fridays 9:00am - 5:30pm, with additional hours for meetings as needed.
POSITION SUMMARY: Under the general direction of the Director of Psychiatry and the Nurse Manager, the Behavioral Health / Psychiatric Registered Nurse actively manages multiple populations with mental and behavioral health needs including those with complex medical and behavioral health care needs. This position is responsible for communication and coordination, including transition care, within and between WCHC and county agencies, hospitals, external providers, APS/CPS, housing, crisis management, Beacon and other insurers, transportation, and other entities as needed. This position is also responsible for population health management of Behavioral Health patients within WCHC.
RESPONSIBILITIES:
- Work as a key member of the care team for complex patients who are part of the behavioral health services at West County Health Centers (WCHC) in the role of a nurse case manager.
- Work closely with behavioral health and psychiatry team members to triage and coordinate care for patients transferring to and from Sonoma County Behavioral Health, hospitals, and other psychiatric facilities.
- Work closely with the WCHC’s behavioral health and psychiatry teams in case consultation for a defined set of patients.
- Assist behavioral health staff in coordinating mental health and other referrals to outside community resources for patients in the Behavioral Health department.
- Monitor, track and facilitate referrals to internal programs, including behavioral health and psychiatric services, in coordination with the Behavioral Health Services Lead/front office staff.
- Make use of Care Team data to identify, connect and manage patients experiencing chronic conditions with needed behavioral and psychiatric services.
- Perform telephone callbacks, prescription refills, ordering labs, etc. as assigned by WCHC behavioral health and psychiatric providers.
- Assist with coordination of patient transportation for referral services when needed.
MINIMUM REQUIREMENTS:
- Current California Registered Nurse License
- Current CPR certification
EXPERIENCE:
- Minimum of two year's experience with direct patient care preferred.
- Bilingual Spanish/English strongly preferred.
- Knowledge of principles of nurse case management for the patient with chronic disease(s).
- Experience working with complex and multiply diagnosed patients preferred.
- Ability to work well on a multidisciplinary team.
- Ability to problem solve and be self-motivated with acute attention to detail.
- Strong organizational skills and the ability to multitask effectively in a busy environment.
- Expected to have strong interpersonal skills and can understand and follow written/verbal instructions.
- Ability to work independently with little or no supervision.
- Excellent interpersonal skills to interact positively with patients and staff alike.
- Proficiency in using office software and electronic health record systems is beneficial.
- Knowledge of principles of nurse case management for the patient with chronic disease.
PHYSICAL REQUIREMENTS:
- Ability to bend and move arms with sufficient range of motion to complete patient related requirements, hearing required for patient assessments, may need to assist patients onto exam table, able to lift 30 lbs., bend, kneel and twist as needed.
- Prolonged standing and occasional sitting required.
- Must be able to perform the essential duties with or without accommodations. Basic office and equipment safety knowledge and practice.
