In this role, you will play a vital part in enhancing the health, recovery, and quality of life of our residents by providing expert nutritional care, developing individualized nutrition plans, and ensuring compliance with all regulatory standards.

Position Summary
The Registered Dietitian is responsible for assessing residents' nutritional needs, developing and implementing individualized nutrition care plans, monitoring outcomes, and collaborating with physicians, nursing staff, speech therapists, and the Food Service Department to ensure optimal nutritional care for residents in a skilled nursing and rehabilitation setting.
Key Responsibilities
Complete comprehensive nutrition assessments for residents upon admission and as required by state and federal regulations.
Develop, implement, and monitor individualized nutrition care plans.
Assess residents for nutritional risk, weight changes, wound healing, hydration status, and special dietary needs.
Recommend therapeutic diets and nutritional interventions based on physician orders and clinical needs.
Monitor residents receiving enteral nutrition, nutritional supplements, and specialized diets.
Collaborate with physicians, nursing staff, Speech-Language Pathologists, and the interdisciplinary care team to optimize resident outcomes.
Participate in resident care conferences and interdisciplinary meetings.
Ensure menus meet nutritional standards and regulatory requirements while accommodating resident preferences.
Work closely with the Food Service Director to ensure quality meal production and therapeutic diet compliance.
Monitor food service operations for regulatory compliance, sanitation, and resident satisfaction.
Educate residents, families, and staff on nutrition-related topics.
Maintain accurate documentation in the electronic medical record.
Ensure compliance with CMS, New York State Department of Health, Joint Commission (if applicable), and facility policies.
Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
Stay current with evidence-based nutrition practices and regulatory updates.
Qualifications
Bachelor's or Master's degree in Nutrition, Dietetics, or a related field from an accredited program.
Registered Dietitian (RD/RDN) REQUIRED
Current New York State Certified Dietitian-Nutritionist (CDN) certification or eligibility required.
Minimum of 1–2 years of experience in long-term care, skilled nursing, rehabilitation, or clinical nutrition preferred.
Knowledge of MDS, wound care nutrition, renal diets, diabetic diets, dysphagia diets, and enteral nutrition preferred.
Strong assessment, documentation, and critical thinking skills.
Excellent communication, organizational, and interpersonal skills.
Proficient with electronic medical records (EMR) and Microsoft Office.
