INTRODUCTION
The Center for Human Development, (CHD) is seeking a Housing Specialist for our Adult Community Clinical Services (ACCS), located in Springfield, MA. ACCS is funded through the Department of Mental Health and is a multidisciplinary outreach approach for persons served in need of psychiatric and other services living independently in the community. This involves active engagement including assertive outreach, consistent assessment and treatment planning, skill building, symptom management, addiction treatment, and behavioral and physical health monitoring and support.
ACCS teams include licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The Team works collaboratively to provide person-centered, evidence-based services in support of Persons’ treatment goals. The Housing Coordinator with Adult Clinical Community Services (ACCS) participants to provide services to obtain and maintain permanent housing. The Housing Coordinator also provides outreach services to a caseload of participants. S/he works within the Agency's team model, establishing open communication, support, and accountability among co-workers.
JOB FUNCTIONS:
- Helps develop client treatment plans
- Initiate contact with participants and engages participants in gathering housing history
- Works in developing a focused strategy for each person or family as it relates to self-sufficiency, permanent housing, and strengthening economic stability
- Assists participants in finding appropriate housing
- Provides tenant education regarding rental and landlord responsibilities
- Provides constant open communication with landlords, building trusting relationships to ensure longevity in permanent housing placements
REQUIREMENTS
- High School Diploma or GED required, Bachelor's degree in a related field (Psychology, Social Work, etc.) preferred
- 1 to 2 years of experience in a mental health-related treatment program
- MAP Certified preferred
- Valid Driver's License and reliable vehicle for work purposes
- Bi-Lingual applicants encouraged to apply
COMPENSATION
