Job Summary
Affinity Care MN is seeking a mission-driven Case Manager to coordinate 245D Home and Community-Based Services for the people we support. The Case Manager assists clients and their families in accessing services, resources, and informal supports; develops and monitors individual service plans; and ensures high-quality, compliant service delivery. This role requires regular community engagement and travel to meet with clients and providers.
Job Responsibilities
- Assist clients and their families in accessing services, resources, and informal supports, including coordinating medical, social, educational, and other services regardless of funding source.
- Develop, implement, and monitor individual Coordinated Service and Support Plans, ensuring all services are current and that providers are enrolled with Minnesota DHS.
- Advocate for clients' needs, respond to Common Entry Point (CEP) reports, and resolve issues with the guidance of a supervisor.
- Maintain required billable hours, complete documentation on time, and ensure client progress is tracked and communicated with interdisciplinary teams.
- Participate in staff meetings and training, adhere to all company and governmental regulations, and manage weekly activities and schedule.
- Comply with MN Statutes Chapter 245D, the Vulnerable Adults Act, and all mandated reporting requirements.
Requirements
Must have one of the following:
- A Bachelor's degree with an emphasis in social services, psychology, human services, or other field related to the education or treatment of people with developmental disabilities or related conditions; OR
- Any Bachelor's degree, plus one year of experience as a social worker, case manager, or care coordinator in a public or private social service agency.
Must also meet the following:
- Valid driver's license and current auto insurance.
- Maintain an acceptable motor vehicle record.
- Must not be disqualified by the required DHS criminal background study.
- Complete all orientation and training as written in policy.
- Good oral and written communication and organizational skills.
- Proficiency with Microsoft Office, Google Workspace, and care-related software platforms.
Preferred Qualifications
- One to two years of previous waiver case management experience.
- Licensed Social Worker (LSW) credential.
- One year of experience providing case management services to persons with developmental disabilities.
Working Conditions
This position travels regularly into the community and works in a variety of settings, both on and off site. The position is subject to frequent interruptions, crisis management, and imposed deadlines. The Case Manager has regular contact with clients, family members, friends, federal, state, and county regulatory personnel, other provider personnel, and the general public, some of whom may be hostile or emotionally upset. The position may be exposed to verbally aggressive individuals, infectious diseases, odor, and dust.
Required Physical and Demonstrated Abilities
- Move intermittently throughout the day.
- Meet the licensing, contractual, and other mandated requirements of the state and county.
- Make independent decisions when circumstances warrant; demonstrate initiative, creative problem solving, and adaptability.
- Follow daily routines while allowing for individual decision-making and flexibility.
- Operate a computer, calculator, and other necessary office equipment.
- Possess a cell phone or maintain adequate phone access to allow for team contact.
