Erie Family Health Centers is seeking a part time (20 hours per week) Community Health RN to provide essential clinical support to patient care activities utilizing nursing education, assessment, and procedural skills.

Compensation is based on each candidate’s experience, skills and education within the range identified for the role. Candidates who meet the minimum requirements of the role will start at entry in the range. Any additional skills, experience and education will be reflected in the compensation offered.
MAIN DUTIES & RESPONSIBILITIES
- Collaborates with providers in the delivery of patient care.
- Performs triage assessment process of walk-in patients and telephone calls.
- Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching.
- Administers immunizations, applicable contraception refills, and medications as ordered by provider.
- Performs other duties as assigned.
Education
Current Illinois RN licensure required.
Current BLS for Healthcare Provider CPR certification required.
Experience
One to two years’ experience in community health nursing or ambulatory care preferred.
