SCHEDULE: Night shift. 36 hours weekly. 7:00pm - 7:30am
ABOUT THE POSITION
In collaboration with the multi-disciplinary Reid Hospice team, the Hospice Case Manager RN is responsible for coordination of all care for their assigned hospice patients; this includes admission, discharge, re-certification, and planning of care. The Case Manager RN will work closely with the Hospice Interdisciplinary Group (IDG), including patients and families, as well as Reid Health Leadership, Medical Staff, and physician’s practice office personnel; will work with representatives of the community, federal, and state agencies, as well as with outside review and accreditation agencies.
