SUMMARY
LifeLong’s Recuperative Care program (Arnold’s Place) is a 50-bed 24/7 residential setting that provides onsite care for patients with complex medical, behavioral, and social needs. Patients receive ongoing medical and behavioral health services, care coordination, meals, and daily supports, along with connections to housing and community resources. Under the general supervision of the Associate Behavioral Health Director, the Behavioral Health Provider will work with an inter-disciplinary team to ensure that quality psychosocial services are delivered to Arnold's Place patients. We are seeking a candidate who is comfortable working in milieu settings and is excited to create and implement various groups and wellness-focused activities to support the overall behavioral health needs of patients living onsite, as well as to provide both individual and group therapy. The Behavioral Health Provider will work collaboratively with all program staff to uphold policies, promote patient well-being, and ensure a respectful and welcoming environment for individuals experiencing homelessness and complex health needs.
This position reports to the Associate Behavioral Health Director.
This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA.

Compensation: LCSW/LMFT/LPCC: $101,753.60 - $122,928.00/year depending on years since licensed and pro-rated to FTE, sign-on bonus, and excellent benefits:

JOB DUTIES:
- Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and implements short-term counseling using appropriate modalities for patients with issues such as: depression, anxiety, trauma, grief/loss, addiction, domestic violence, behavioral health problems, and psychosocial stressors. Works predominantly within clinic, but may include home visit services.
- Provides short-term case management services for patients participating in treatment.
- As a team member, assists with crisis intervention and triage as needed. Crisis intervention includes assessment, de-escalation, and notifying the public mental health crisis response team if necessary.
- Works with team members to provide follow-up assessment and prevention planning after any crisis, including follow-up with outside agencies.
- Maintains clinical and administrative records in accordance with contractual and billing requirements and submits encounter forms on a weekly basis.
- Participates in weekly clinical meetings; promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.
- Participates in quality improvement efforts related to behavioral health and integrated models of care.
- Consults informally with providers as needed.
- Follows all State of California mandated reporting requirements.
- May serve as supervisor for psychology or PCC/MFT/MSW interns and may provide APCC/AMFT/ASW clinical supervision.
JOB QUALIFICATIONS:
- Ability to work effectively and collaboratively in a fast-paced primary health care setting and prioritize completing job demands.
- Must be a self-starter, able to balance counseling, case management and administrative tasks.
- Demonstrated ability and sensitivity to working with a diverse staff and with a patient population from various ethnic, socioeconomic, age, education, lifestyle, and experiential backgrounds.
- Excellent verbal, written and interpersonal communication skills. Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
- Ability to effectively present information to others, including other employees, community partners and vendors.
- Ability to seek direction/approval from on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
- Work in a team-oriented environment with a number of professionals with different work styles and support needs.
- Ability to effectively supervise staff and to contribute to staff development.
- Conduct oneself in external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
- Ability to see how one’s work intersects with that of other departments of LifeLong Medical Care and that of other partner organizations.

Job Requirements:
- Current and valid California LCSW, LMFT, or LPCC license with the California Board of Behavioral Sciences.
- 1 year experience in clinical case management.
- Knowledge and experience providing mental health services to disadvantaged populations.
- Familiarity with evidence-based practices for treatment of depression and other mental disorders.
- Proficient in Microsoft office.
