INTRODUCTION
LifeLong Medical Care has an exciting opportunity for a nurse to join our multi-disciplinary team providing primary care services at LifeLong Medical Care. The ideal candidate must have a commitment to providing high quality primary care for complex patients, dedicated to building a collaborative team model of care, and experience working with behavioral health diagnoses.
Under the general supervision of the Associate Medical Director and Center Manager, with clinical guidance of the Director of Nursing, the Nurse Supervisor is responsible for providing clinical care, administrative structure and overseeing the work of Registered Nurses and Medical Assistants within the health center. The nurse will participate in telephonic and in person patient triage, education and counseling, participating in the ongoing education of the care team and collaborating with medical providers to maximize quality of care for our patient populations as well as nurture professional development of RN’s and MA’s.

Compensation: $121,534.40 - $144,248.00 per year, depending on years licensed. ESSENTIAL FUNCTIONS
Clinically, in addition to clinical duties as an RN, the nurse supervisor is responsible for:
- Learning Lifelong-wide approved standardized procedures; reviewing standardized procedures with RN’s; supervise implementation of standardized procedures.
- Regular teaching for RN’s regarding specialized topics such as chronic care management, wound care, triage of urgent concerns, motivational interviewing, lifestyle counseling based on LifeLong accepted evidence based practice.
- Developing protocols for common abnormal labs for further work-up prior to provider review.
- Regular trainings for medical assistants regarding healthcare maintenance screenings, vaccine schedules, blood pressure readings, and other tests performed by MAs.
- Quality improvement oversight for MA’s, including Tableau workplaces.
- Mentoring for RN and MA specialized projects.
- Providing clinical oversight for triage team: develop protocols for triage to refer to appropriate clinical staff for patient concerns.
- Represent the clinic at LifeLong-wide RN supervisor meetings to address clinical concerns, content areas for education and implementing new protocols.
- Coordinate with Director of Nursing to provide feedback on staffing needs, clinical workflows, and support organization-wide innovation projects as assigned by DON.
- Serve as back up for triage team and ensure timely response to clinical needs when a team member is absent (including triage assistant, MAC, RN).
Administratively, the nurse supervisor is responsible for:
- Monthly meetings with RNs to discuss workflows.
- Daily RN schedule, including which RNs are designated for walk-ins, new patient evaluations, medication refills, and task box.
- Advocate on behalf of the RN/MA staff to providers and receive feedback about the clinical performance of RN/MAs from provider level staff.
- Monitoring the timely and appropriate completion of tasks by RNs.
- Delegation of tasks among RNs.
- Follow up on incident reports to improve safety and patient care within the clinic.
- Provide oversight and act as a resource for point of care testing and maintenance, medication stock inventory and appropriate stocking of clinical supplies including emergency kits.
- Work closely with the Center Manager to ensure the site is adequately stocked and prepared for emergencies and/or disasters.
- Upon hire, annually and as needed: evaluation of core competencies for individual RNs.
- Regular evaluation of core competencies for individual MAs, and supervision of MA workflows for healthcare maintenance, vaccinations, and other quality improvement measures.
QUALIFICATIONS
- Knowledge of nursing and medical practices, procedures, regulations, and guidelines pertaining to patient care.
- Knowledge of theory, methods and techniques required for rendering patient care.
- Ability to seek direction/approval from supervisor on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
- Work in a team-oriented environment with a number of professionals with different work styles and support needs.
- Excellent interpersonal, verbal, and written skills and ability to effectively work with people from diverse backgrounds and be culturally sensitive.
- Experience working with diverse patients including the homeless population, disabled persons, and individuals with co-occurring disorders.
- Commitment to the provision of primary care services for the underserved with demonstrated ability and sensitivity in working with a variety of people from low-income populations, with diverse educational, lifestyle, ethnic and cultural origins.
- Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.

- Current and valid California Registered Nurse license and BLS.
- Proficient in Microsoft office suite.
