BASIC QUALIFICATIONS
- Graduate of an accredited school of nursing, Current NYS RN License required.
- Previous RN experience is preferred. New Graduate RN's welcome to apply.
- Previous Home Health Care experience preferred.
- Minimum 1 year of medical-surgical experience.
- BLS required.
- ACLS preferred.
- Valid NYS Driver's license and proof of auto insurance.
- Must be available weekends and Holidays and provide on-call services as required.
ROLE AND RESPONSIBILITIES
- Planning, coordinating, providing and documenting the care of patients in their place of residence.
- Clinical practitioner who coordinates and implements patient care specific to Jefferson County.
- Ensures that compassionate, engaged care meets or exceeds quality standards.
- Care is provided in an efficient and safe manner consistent with the NYS licensure and Samaritan Home Health standard of care.
- Utilizes nursing skills to determine the biophysical, psychosocial, environmental, self-care, cultural, ethnic and spiritual and educational needs of the patient/family/significant other to develop, implement and revise the plan of care. In doing so, performs accurate assessments, interprets and correlates finding, establishes plan of care and implements short and long-term goals.
- The nurse observes patient progress, records pertinent observations, implements physician and nursing treatment plans and evaluates outcomes.
- The RN recognizes their responsibilities as specified by law and abides within those responsibilities.
- Provides direct supervision to LPNs and HHAs.
- Provides on-call services as required.
