JOB DETAILS
Job Duties:
- Assess, plan, develop and provide the patient or responsible caregiver a home health plan of care for occupational therapy.
- Identifies patient needs for other disciplines and services.
- Coordinate and evaluate the plan of care with all other disciplines, organizations and physicians for an assigned caseload.
- Communicates significant changes in patient status to the physician, rehab coordinator and the appropriate agency personnel when indicated.
- Demonstrates effective communication skills, keeping lines of communication open between Lead O.T. Supervisor, Team Managers and all staff involved in care of assigned patients.
MINIMUM REQUIREMENTS:
- 2 years of experience as an occupational therapist
- Current registration to practice Occupational Therapy in New York State
COMPENSATION
- Posted Salary Range: USD $90,000.00 - USD $115,000.00 /Yr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
