Key Responsibilities:
- Perform nursing assessments and evaluate client/patient physical conditions to determine service needs.
- Create and implement action plans based on clients' physical and mental health treatment needs.
- Monitor health status and coordinate care with the treatment team to ensure comprehensive support.
- Administer medications and oversee compliance with state regulatory standards.
- Provide health education and disease prevention strategies to clients and their families.
- Arrange medical appointments and manage emergencies, advocating for necessary medical services.
- Maintain accurate and timely documentation in accordance with company policies.
Education, Experience, and/or Credential Qualifications:
- Current Registered Nurse licensed in the state of practice.
- Must be willing to become licensed in additional states.
- Must be comfortable providing telehealth services.
- BCLS certification required; ACLS certification preferred.
Additional Qualifications:
- Successful completion of background check including criminal record, driving record, abuse/neglect, and fingerprint check.
- Current driver’s license, acceptable driving record, and current auto insurance.
