Waverly Gardens is seeking an On-Call Occupational Therapist.

Responsibilities
As an Occupational Therapist you will be responsible to:
- Conduct a comprehensive assessment & evaluation using subjective and objective measures.
- Develop treatment plans and programs in accordance with physician orders.
- Provide quality treatments, actively progressing goals to achieve highest quality of life.
- Conduct screens and provide education to residents/clients, families, caregivers, and staff.
- Coordinate seamless discharges utilizing the full continuum of therapy and fitness services.
- Provide accurate and timely documentation & billing in electronic medical record.
- Demonstrate understanding and knowledge of payer sources and regulatory requirements.
- Complete and document supervisory visits per regulations.
- Communicate effectively with residents/clients, families, caregivers, physicians, referral sources, and interdisciplinary team.
QUALIFICATIONS
- Graduate from an accredited Occupational Therapy program.
- Current licensure in the state(s) of practice as required.
- Demonstrated compatibility with PHS's mission and operating philosophies.
- Demonstrated ability to read, write, speak and understand the English language to communicate with all customers.
PAY RANGE
The estimated starting pay range for this role is $36.79 - $43.28 per hour. This good faith estimate is based upon PHS’s typical starting pay range for this role. Actual pay range depends upon many factors including each individual employee’s qualifications and market conditions and is subject to change.
