Responsibilities:
- Provide personal care while assisting with showers, grooming, feeding, and medication management, as needed while assisting with daily living activities, to ensure the comfort and well-being of our clients
- Keeping up with light housekeeping, running errands or shopping and transporting or escorting clients to appointments
- Encourage independence and participation in activities, while promoting a safe and welcoming environment
- Communicate effectively with clients, family members, and interdisciplinary team members to ensure coordinated care and support
- Maintain accurate records of care and services provided
- Participate in ongoing training and education to enhance knowledge and skills
