Verify your credentials meet training plan standards
Your DS-2019 training plan must align your prior nursing assistant education or certification with the specific clinical skills you'll develop in the U.S. Document your home-country credentials, any CNA coursework, and hands-on hours before approaching a designated sponsor.
Target host employers with active J-1 training history
Search Migrate Mate to identify U.S. healthcare facilities that have hosted J-1 Trainees or Interns before. Hospitals and skilled nursing facilities with established training programs are far more likely to navigate the DS-2019 process alongside a designated sponsor than those encountering it for the first time.
Clarify the 2-year home residency requirement early
Nursing Assistants from countries with exchange agreements, or funded by their home government, may be subject to a 2-year home-country residency requirement after program completion. Confirm your status before accepting a host offer, since this affects any future U.S. work visa plans.
Align your role with the correct J-1 category
Use the Intern category if you're a current student or graduated within 12 months. Use the Trainee category if you graduated more than a year ago or have relevant work experience. Mismatched categories cause DS-2019 rejections, so confirm the fit with your designated sponsor before your host employer finalizes the offer.
Confirm your host employer's E-Verify enrollment
J-1 exchange visitors are authorized to work only at the specific host employer listed on your DS-2019. Confirm that your prospective host is enrolled in E-Verify and understands that you cannot pick up supplemental shifts at other facilities without written sponsor approval and a DS-2019 amendment.
Request a training plan draft before the offer stage
Designated sponsors require a detailed Training Plan form (DS-7002) signed by the host employer before issuing your DS-2019. Ask your prospective host to draft this document during the interview process, not after the offer, so delays don't push back your program start date.

Find Nursing Assistant JobsNursing Assistant J-1 Visa: Frequently Asked Questions
Which J-1 program category applies to Nursing Assistant roles?
The J-1 Intern category applies if you're currently enrolled in a post-secondary nursing or healthcare program, or graduated within the past 12 months. The J-1 Trainee category applies if you graduated more than a year ago or have prior work experience in a clinical support role. Both categories require a U.S. Department of State-designated sponsor to issue your DS-2019, not the hospital or care facility where you'll train.
Who actually sponsors my J-1 visa as a Nursing Assistant?
Your J-1 visa sponsor is a U.S. Department of State-designated organization, such as CIEE, Cultural Vistas, or AIPT, not your host employer. The host facility is your training site. The designated sponsor issues your DS-2019, monitors your program compliance, and signs your Training Plan. Conflating the two is a common mistake that leads to mismatched expectations during the offer process.
How do I find U.S. healthcare employers that accept J-1 Trainees or Interns?
Migrate Mate lets you filter for U.S. employers and Nursing Assistant roles that align with J-1 exchange visitor programs, so you're not cold-applying to facilities unfamiliar with the DS-2019 process. Working with hosts that already understand J-1 program requirements reduces delays between your offer and your program start date significantly.
Does the J-1 visa for Nursing Assistants include a 2-year home residency requirement?
It can. If your J-1 program is funded by your home government, if your home country appears on the State Department's skills list, or if your exchange agreement triggers the requirement, you'll need to return home for two years after your program before qualifying for an H-1B, L-1, or immigrant visa. Confirm your specific situation with your designated sponsor before accepting any host offer.
Can I work at multiple healthcare facilities during my J-1 program?
No. Your J-1 work authorization is tied to the single host employer named on your DS-2019. Working additional shifts at other hospitals, clinics, or care facilities, even in the same Nursing Assistant capacity, requires a formal DS-2019 amendment approved by your designated sponsor in advance. Unauthorized secondary employment is a program violation that can result in SEVIS termination.
