Responsibilities
- Carry out and maintain best practices for confidentiality and safekeeping of all patient records and information related to treatment and services, with up-to-date working knowledge of confidentiality statutes (HIPPA, 42CFR).
- Work effectively as a Multi-Disciplinary Team member across all departments; embrace complex communication and understand the process of change to enhance the working relationship among all departments and professionals.
- Work closely with Blueprint RN to provide access to chronic disease management, including self-management support to individuals and their families with referrals to community, social support, and recovery services as needed.
- Document all encounters and patient services to ensure accuracy of coding/billing.
- Maintain professionalism in the workplace including professional boundaries, follow chain of command and codes of ethics.
- Participate in required practice staff meetings, as well as clinical and administrative supervision meetings as requested.
- Act as patient advocate to coordinate required services or to resolve emergency problems in crisis situations.
- Remain in compliance with all practice policies and procedures as they pertain to patient’s engagement in treatment are upheld.
- Review patient progress at start, mid-point and end of treatment and update care plans to reflect changes in care plan goals and recommendations.
- Provide outreach, clinical counseling services, home visits and group therapy to support patients in their recovery, as needed.
- Attend community collaborative, planning, and Blueprint for Health meetings and trainings, as deemed necessary by HSA program manager, to ensure engagement with community and state priorities related to Hub and Spoke programming and to meet HSA community health needs.
- Perform other duties as requested.
MINIMUM EDUCATION:
Master's degree in social service related field (MSW or LMHC).
MINIMUM WORK EXPERIENCE:
- Knowledge and experience with behavioral health and patients with addiction preferred.
- Two years’ experience preferred working in a health care setting, social services agency or similar organization required.

Current non-restricted.
