Ascension Saint Thomas Rehabilitation Hospital is a 40-bed acute rehabilitation hospital located in Nashville, TN and is part of Lifepoint Health, a diversified healthcare delivery network committed to making communities healthier ® with acute care, rehabilitation, and behavioral health facilities from coast to coast. What We’re Looking For
The Licensed Practical Nurse (LPN) provides hands-on nursing care under the supervision of a Registered Nurse (RN). This includes delivering treatments, monitoring patient progress, supporting care planning, and ensuring safety and comfort in alignment with individualized patient needs.
Essential Functions
- Administer medications safely, accurately, and based on patient age and clinical condition.
- Minimize potential for medication errors by following proper medication administration procedures.
- Assist the RN with initial assessments and patient reassessments per policy.
- Identify and implement appropriate rehabilitation nursing interventions.
- Perform timely and accurate Quality Improvement (QI) assessments under RN supervision.
- Deliver care based on patient response to treatment and update the interdisciplinary plan accordingly.
- Assess and reassess pain; use appropriate pain management techniques and educate patients and families.
- Perform all patient care tasks in a manner that promotes patient safety and reduces medical errors.
- Develop and implement individualized teaching plans based on patient and family learning needs.
- Perform other duties as assigned.
Applicants should have a current state LPN license from an accredited nursing school. Additional requirements include:
- Education: Graduate of an accredited Licensed Practical/Vocational Nurse program.
- Experience: Minimum of six months of Med/Surg experience in an acute care setting preferred.
- License: Current state licensure as a Licensed Practical/Vocational Nurse.
- Certifications: CPR/BLS certification required; IV Certification highly preferred or obtained within 6 months.
- Strong clinical skills and attention to detail.
- Compassionate approach and commitment to patient-centered care.
- Ability to work effectively within an interdisciplinary team.
