We offer medical services including Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, and Home Health Aide care to those who wish to remain safely in their home. Position
Clover Home Health has multiple openings for a part-time Home Health Registered Nurse. This position is on the road, meeting with clients in their homes in the Greater Indianapolis area.
Ideal Candidate
Our ideal Home Health Registered Nurse candidate must have OASIS experience,EMR systems Axxess (preferred but not required) and 2 years in Home Health Care.* The Home Health Registered Nurse* candidate must have strong written and communication skills, be able to work independently and collaborate effectively with our DON/ADON. The Home Health Registered Nurse candidate must embody our Clover Values of TEAMWORK, CONSISTENCY, COMPASSION, AND GRATITUDE. We are looking for someone who understands the importance of care and has the heart of a clinician!
Role
- Complete new patient intakes
- Direct patient care (SOC's, ROC; Re-Certs, Discharge, Transfers, Nurse Visits, and ALL OASIS documentation)
- Document complex medical information to outline and improve health results that help determine immediate and routine care for the client
- See patients and clients, as needed
- Work collaboratively with the DON/ADON
- Communicate patient conditions and collaborate with the DON/ADON to deliver care when patient needs evolve
- Work to decrease re-admissions by promoting preventative care and ensuring continuity of care
- Additional duties as assigned
Requirements
- Degree in Nursing from an accredited nursing school, university or college
- Valid driver's license and automobile insurance
- Current in-person CPR certification
- TB test screening (past 12 months)
- Two years Home Health based nursing experience (required)
- Two years OASIS experience (required)
- Tech savvy, able to learn and navigate several platforms. Proficient with Apple products.
- Perform routine duties independent of supervision but consults with others as required
- Excellent communication and listening skills
- Use of AXXESS, EMR system (preferred)
- Excellent time management (required)
