Overview
Registered Nurse with experience in home health and patient-centered care coordination. Skilled in comprehensive assessments (OASIS), care plan development, and interdisciplinary collaboration to improve patient outcomes. Proven ability to manage complex cases, support transitions of care, and educate patients and families while ensuring compliance with clinical and regulatory standards.

Duties

· Reviews client referral information and responds to requests and inquiries as appropriate.

· Performs initial and ongoing client assessments (OASIS) based on Agency policy and standards of practice to ensure effective and appropriate home care services.

· Directs, plans, and initiates appropriate action independently and responsively in
home care situations.

· Coordinate care across interdisciplinary teams including physicians, therapists, social workers, and community resources

· Manages/supervises a team of RNs, LPNs, Home Health Aides, and Personal Care Aides to provide effective and quality home care services.

· Participates in human resource management to achieve quality service delivery and positive employee relations.

· Promotes personal safety and a safe environment for clients and coworkers.

· Performs job duties in accordance with Agency policies, procedures, and professional and community standards.

Qualifications

· Active Registered Nurse (RN) license in the state of Indiana

· 1+ years clinical experience in home health, Med-Surg, urgent care, or long-term care settings preferred

· Strong knowledge of care coordination and transitions of care

· Experience with OASIS (Outcome and Assessment Information Set) assessments preferred

· Demonstrated ability to perform comprehensive patient assessments and develop individualized care plans

· Proficiency in utilization review and ensuring medical necessity of services

· Ability to work independently while managing a diverse patient caseload across multiple locations

· Excellent communication and interpersonal skills for collaboration with patients, families, and interdisciplinary teams

· Strong documentation skills with experience in electronic health records (EHR systems)

· Knowledge of Medicare/Medicaid guidelines and home health regulations preferred

· Compassionate, patient-centered approach with a focus on improving health outcomes

· Valid driver’s license and reliable transportation for travel within service area
