What You’ll Do:
The Care Manager supports an assigned caseload of BH/IDD Tailored Plan members, including assessment of member needs, linking members to network and community resources, developing and monitoring plans of care, participating with care teams, and addressing the barriers which may be interfering with the member's desired outcomes for a community-involved quality life.

Education AND Experience
- A bachelor’s degree in a field related to health, psychology, sociology, social work, nursing, or another relevant human services area, or licensure as a registered nurse (RN)
- Two years of experience working directly with individuals with behavioral health conditions
- Valid Driver's license in state of residency.
