About the Program
This is an exciting opportunity to be a part of Metro-Boston’s PACT-Y team through the Home for Little Wanderers. PACT (Program for Assertive Community Treatment) is an evidence-based home/community centered model.
The PACT-Y Outreach/Case Manager provides intensive, community-based support to youth with significant emotional and behavioral health needs as part of a multidisciplinary team. This role focuses on engaging youth and families in services, supporting individualized treatment goals, and coordinating care across systems.
This is a highly collaborative, field-based position that requires flexibility, strong relationship-building skills, and the ability to adapt to the evolving needs of youth and families.
Key Responsibilities
- Deliver community-based outreach and case management services in home, school, and other natural settings
- Engage youth and caregivers to build consistent, trusting relationships
- Support implementation of individualized treatment plans in partnership with the team
- Assist with crisis prevention and intervention, utilizing a team-based approach
- Coordinate care with providers, including mental health, medical, educational, and state agencies
- Support development of life skills, emotional regulation, and social functioning
- Connect youth and families to community resources and supports
- Participate in daily team meetings and ongoing treatment planning
- Complete timely and accurate documentation in compliance with program standards
- Provide transportation support for youth as needed
Schedule: Full-time with flexible hours including evenings and occasional weekends.
Qualifications
- High School diploma or GED. Bachelor’s degree preferred.
- Must be able to work with people of different backgrounds, languages and ethnicities.
- Driver’s license required.
