Compensation: $85,000 Annually + Performance-Based Bonus Opportunities

Position Overview

Help older adults get the mental health care they deserve. Bespoke Psychiatry is seeking an experienced and highly motivated Behavioral Health Care Manager (BHCM) to provide clinical leadership, oversee care coordination, and support the delivery of high-quality behavioral health services across our network of skilled nursing and assisted living facilities.

This position plays a key role in our Behavioral Health Integration (BHI) and Chronic Care Management (CCM) programs, working collaboratively with psychiatrists, psychiatric nurse practitioners, physician assistants, medical assistants, and other healthcare professionals.

The ideal candidate is a licensed behavioral health professional with strong clinical judgment, proven leadership experience, and the ability to manage both patient care responsibilities and team operations in a fast-paced healthcare environment.

Job Responsibilities

• Clinical care management: Oversee behavioral health care management, including patient assessments, individualized care planning, clinical interventions, caseload coordination, crisis response, and collaboration with psychiatric providers to ensure quality care and compliance with clinical standards.
• Leadership and MA team oversight : Lead and supervise the medical assistant team by overseeing daily workflows, assigning tasks, monitoring performance and documentation accuracy, providing training and coaching, and ensuring compliance with established clinical protocols and standards.
• Collateral, family and PCP communication: Maintain effective bilingual communication with facility staff, patients, families, and primary care providers, ensuring timely updates, accurate documentation, and coordinated patient care in compliance with confidentiality and HIPAA requirements.
• Documentation, compliance and billing readiness: Maintain accurate and timely clinical documentation, manage patient records and care management time tracking, ensure billing readiness, and uphold compliance with HIPAA, privacy regulations, and established documentation standards.
• Quality and reporting: Conduct regular case reviews, monitor key performance indicators, prepare reports, and identify opportunities for quality improvement to enhance patient care and program effectiveness.
• Training: Complete required onboarding and ongoing professional development, including continuing education in behavioral health, while providing training and guidance to medical assistants to ensure competency in clinical assessments, care management activities, and established protocols.

Required Qualifications

• Active, unrestricted Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) license.
• Bilingual proficiency in English and Spanish is required.
• Minimum of 2 years of clinical leadership or supervisory experience in behavioral health, care management, or a related healthcare setting.
• Strong understanding of behavioral health assessments, treatment planning, crisis intervention, and clinical care coordination.
• Demonstrated ability to supervise staff, manage workflows, and maintain accountability for performance and quality standards.
• Excellent organizational, communication, critical-thinking, and problem-solving skills.
• Ability to work independently, prioritize competing responsibilities, and meet established deadlines.
• Valid driver's license, reliable transportation, and willingness to travel to facilities.

Preferred Qualifications

• Experience working in skilled nursing facilities, assisted living communities, or long-term care settings.
• Experience supporting older adults with complex psychiatric or behavioral health conditions.
• Familiarity with Behavioral Health Integration (BHI), Chronic Care Management (CCM), or collaborative care programs.
• Experience with motivational interviewing, brief therapeutic interventions, and standardized behavioral health screening tools.
• Experience with electronic health records, telehealth platforms, and clinical documentation requirements.
• Experience working with individuals with substance use disorders.
