PCHC’s Care Management team is seeking an experienced, mission-driven Medical Assistant to join us as a Medicare Annual Wellness Visit Specialist – FLOAT. In this unique role, you’ll provide direct patient care while supporting access to preventative services across multiple PCHC locations. You’ll identify patients who are overdue for wellness visits or screenings, reach out with compassion, and help close critical gaps in care. SIGN ON BONUS
All full-time, externally hired Medical Assistants will receive a $3500 sign on /retention bonus!
SCHEDULE
Full-Time, Monday-Friday, Typically 7:30am-4pm | Some schedule flexibility may be required for coverage needs.
(This position requires regular travel between PCHC sites. Mileage for any travel between sites will be reimbursed in accordance with PCHC’s Business Travel & Expense Reimbursement policy.)
SALARY
Ranges from $21-30/hour depending on qualifications, skills, and years of relevant experience.
HIGHLIGHTS OF THE POSITION
- Demonstrates compassion, collaboration, communication, and appropriate clinical judgement in patient care.
- Responsible for conducting pre-planning planning, outreach, scheduling, and verification activities for preventative services and Medicare Annual Wellness visits.
- Performs Medicare Wellness visits with identified patients in person or via telehealth and collaborates with providers for documentation, review, and sign off of completion.
- Is educated on chronic disease management and preventative care, proving related teaching to patients as needed. Identifies chronic diseases that need to be addressed at visit.
- Conducts patient chart reviews for pre-visit planning, medication refills, and other clinical requests. Supports clinical desktop work.
- Is flexible in adapting to the needs of the department, stepping in to perform clinical duties and/or procedures within scope of licensure.
- Creates care plans and strategizes within care team to effectively manage patient’s care.
- Assists in maintaining compliance with quality assurance standards.
- Performs all function of the Medical Assistant role as outlined in PCHC’s Medical Assistant job description and Clinical Competency Checklist.

EDUCATION AND EXPERIENCE
- High School Diploma (or equivalent) required. Relevant postsecondary education preferred.
- Minimum of two years’ experience as a Medical Assistant in primary healthcare or related setting required.
- AAMA Certified Medical Assistant certification preferred.
- Will have up to-date training and certification in Healthcare Provider Basic Life Support issued by American Heart Association, American Red Cross, or American Health & Safety Institute. Must obtain within 2 weeks if not current at hire.
