Job Title: Community Outreach Worker
Department: Operations
Location: Newark Wayne Community Hospital
Address: 1200 Driving Park Ave, Newark, NY
Hours Per Week: 24 or more
Schedule: Most usually scheduled for Four 6-hour daytime shifts, could include weekends and sometimes includes attending events in-person.
SUMMARY
The Outreach Worker will support the Program Manager with the coordination of community-based support and engagement services to individuals and families throughout Wayne County. The Outreach Worker will manage established partnerships to ensure schedules; referrals and marketing protocol are supported. The Outreach Worker ensures events are visible on all platforms, including but not limited to, websites, social media and print with distribution to local agencies, health centers, retailers and town chambers.
Responsibilities:
- Conduct outreach activities to identify and engage individuals in need of services.
- Develop and maintain relationships with local organizations, agencies, and community leaders.
- Organize and participate in community events, workshops, and educational sessions.
- Maintain accurate records and submit required reports in compliance with grant requirements.
- Travel throughout rural areas to attend community meetings.
- Provide information and referrals to community resources, health programs, and social services.
REQUIRED QUALIFICATIONS:
- Associate’s degree
PREFERRED QUALIFICATIONS:
- 3 years of administrative office experience in healthcare.
- Excellent oral and written communication and interpersonal skills.
- Knowledge of rural community resources and challenges.
- Strong organizational and problem-solving skills.
- High level of proficiency with PC, software applications, and reporting tools.

Physical Requirements:
Sedentary Work - Exerting up to 10 pounds of force occasionally. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
