Position: Individual Support Specialist - Site Based
Block Institute is seeking an experienced and dedicated Individual Support Specialist to join our Adult Day Services team. In this role, you will collaborate with the interdisciplinary team to develop and implement day program plans based on person-centered practices. You'll be responsible for managing electronic case records through THERAP and ensuring the success and well-being of individuals in the program. Essential Job Duties and Responsibilities:
- Maintain and review electronic case records, ensuring compliance with Block Institute's policies and protocols.
- Develop individual program plans, including assessment, development, implementation, and monitoring of training programs.
- Create and update Day Hab Staff Action Plans semi-annually, and as necessary.
- Conduct semi-annual Life Planning Team meetings and ensure thorough documentation of goals, outcomes, and program adjustments.
- Train Direct Support Professionals (DSPs) on the implementation of services and support to ensure compliance with all OPWDD regulations and Block Institute's standards.
- Collaborate with the Enrollment and Compliance Coordinator to ensure billing documentation corresponds with services listed on the Day Habilitation Staff Action Plan.
- Prepare monthly progress notes and submit billing documentation to the finance department.
- Monitor progress toward individual goals and update staff action plans as needed.
- Conduct weekly email updates to ADS leadership, providing detailed information about participants' progress.
- Ensure the health, safety, comfort, and well-being of program participants at all times.
- Maintain confidentiality of program participants and follow Block Institute's policies on reporting incidents, including abuse and neglect.
- Use positive approaches in difficult situations and demonstrate flexibility and adaptability to change.
- Adhere to the Justice Center Code of Conduct and Block Institute's policies, including attendance and punctuality.
- Represent Block Institute in a professional manner, upholding the agency's mission, vision, and values.
Qualifications
- Bachelor's degree in health, human services, or special education. Master's degree preferred.
- Minimum three years' experience working with individuals with intellectual disabilities.
- Supervisory and training experience preferred.
- Working knowledge of NYS OPWDD Day program regulations.
- Proficiency in Microsoft Word, Excel, and Therap software.
- Training in the Council for Quality Leadership’s Personal Outcome Measures evaluation tool preferred.
