Description
The ECMO/Clinical Research Fellowship is based out of the Division of Pediatric Surgery as well as the Department of Pediatrics. This is a non-ACGME accredited clinical and research fellowship. The program offers specialized training in the principles and practice of multidisciplinary care of patients requiring extracorporeal membrane oxygenation (ECMO), ventricular assist devices (VADs) and extracorporeal carbon dioxide removal for respiratory and cardiac failure.
Qualifications
- Currently enrolled in a general surgery postgraduate training program or
- Completed a general surgery postgraduate training program or
- Completed a pediatric critical care fellowship
- Current physician licensee in the State of Georgia.
- Member of Children's Healthcare of Atlanta Professional Staff in good standing
Educational Program:
The goal of the ECMO fellowship is to provide exceptional training in the identification, management and treatment of pediatric patients requiring ECMO. The fellowship is designed to be tailored to the particular needs of our trainees. All fellows will receive broad-based training that includes rotations within the cardiac, neonatal, and pediatric ICUs; instruction in priming, maintenance and troubleshooting of extracorporeal circuits; training in renal replacement therapies, transfusion medicine, and relevant imaging technologies; and supervising the medical and surgical components of cannulation.
Supervision
The position of fellow shall involve a combination of supervised, progressively more complex and independent patient evaluation and management functions and formal educational activities. The competence of the fellow shall be evaluated on a regular basis by the Fellowship Program Director.
Scope of Practice
- The fellow will provide patient care under the supervision of an Active member of Children’s Professional Staff. The attending physicians maintain responsibility for all activities performed by the fellow.
- The fellow has the following responsibilities:
- Maintain responsibility, within the scope of current professional competence, for the care and supervision of assigned patients.
- Provide the patients with care at the generally recognized professional level of quality and efficiency established for Children’s Healthcare of Atlanta.
- Write orders for treatment under the direction of or to implement the plan of treatment of the Professional Staff member responsible for the child’s care. The attending physician is not required to co-sign orders written by the fellow.
- Abide by the policies and procedures of Children’s and the Rules and Regulations of the Professional Staff.
- Abide by the ethical principles of his/her profession.
- Participate in the quality management/improvement activities of Children’s.
Competency Based Goals and Objectives:
Patient Care:
The ECMO Fellow will assume increasing responsibilities in the management of pediatric and neonatal ECMO patients. While the attending physicians maintain responsibility for all activities performed by the fellow, each fellow may independently perform the following procedures and evaluations without an attending physician present:
- Perform complete admission and consult history and physical examination.
- Write admission and daily orders.
- Provide appropriate documentation in the medical record.
- Dictate medical records in a timely manner as outlined by Children's Healthcare of Atlanta.
- Communicate effectively with patients and their families.
- Communicate effectively with all members of the health care team (including social workers; nutritionists; attending physicians; respiratory therapists; nurses; colleagues; and others).
- Assess radiographic images.
- Propose a treatment and/or workup plan.
- Present a complete clinical picture to the attending physician, who provides further guidance and modification.
- Evaluate children in the ED and inpatient setting with ability to perform ED procedures and make decisions for urgent/emergent intervention.
Inpatient Clinical Care:
Fellow is expected to:
- Thoroughly evaluate all ECMO consults and potential ECMO candidates.
- Be present for all PICU and NICU ECMO cannulations.
- Make rounds and co-manage with the NICU, PICU and CICU all patients on ECMO or with a VAD. This requires comprehensive knowledge of the patient’s status, knowledge of all laboratory tests, radiographs, and vital signs as well as clinical course.
- Summarize clinical care and devise treatment plans.
- Troubleshoot ECMO circuits and patient problems when they arise, working together with the ECMO team, NICU, PICU and/or CICU.
- Effectively communicate and work in a team with NPs, residents, fellows, attendings, ECMO techs and others involved in patient care. This includes following through on any ordered tests, assessing the results, and devising follow up plans.
- Attending ECMO follow up rounds, assessing needs for discharge and participating in ECMO follow up clinic.
