Position Summary
The Licensed Practical Nurse (LPN) provides skilled nursing services to individuals with Intellectual and Developmental Disabilities (IDD) in home and community-based settings throughout Pennsylvania. Working under the supervision of a Registered Nurse (RN) Case Manager, the LPN implements individualized care plans, administers medications, performs clinical procedures, and supports individuals and families in maintaining health, safety, dignity, and quality of life.
Position Scope
This position operates within the Nursing Department and reports directly to the RN Case Manager or Director of Nursing. The LPN delivers hands-on clinical care consistent with Pennsylvania regulations and OneWell Health Care standards. Services are provided in participant homes, community settings, and other approved service environments.
Key Responsibilities
The Licensed Practical Nurse (LPN) is responsible for:
- Implementing individualized care plans
- Administering prescribed medications and treatments
- Performing clinical procedures including wound care, catheter care, feeding tube management, and vital sign monitoring
- Supporting individuals with chronic condition management such as diabetes, epilepsy, and respiratory conditions
- Monitoring and documenting physical, emotional, and behavioral changes
- Promptly reporting changes in condition to the RN Case Manager
- Maintaining accurate, timely documentation in compliance with regulatory and organizational standards
- Providing education and training to families, caregivers, and new nursing staff
- Participating in interdisciplinary meetings and case reviews
- Maintaining infection control and safety practices in all care environments
- Supporting quality improvement initiatives
- Assisting with internal audits and clinical compliance reviews
- Participating in required training and professional development
- Performing additional duties as assigned within the scope of licensure to ensure coordinated, compliant, and person-centered care delivery

Confidentiality
The LPN is responsible for safeguarding all Protected Health Information (PHI) and confidential participant information in accordance with HIPAA, state law, and OneWell Health Care policies. Unauthorized disclosure of confidential information is strictly prohibited.
Documentation Requirements
- Maintain accurate and timely clinical documentation in the designated electronic health record system.
- Document all medications, treatments, assessments, and incidents.
- Ensure documentation aligns with ISP requirements and nursing standards.
- Complete reports in compliance with state and organizational timelines.
Certifications
- Current and active Licensed Practical Nurse (LPN) license in Pennsylvania
- CPR and First Aid certification (required)
- Any additional certifications required by state or waiver programs
Education
Completion of an accredited Practical Nursing program approved by the Pennsylvania State Board of Nursing.
Experience
Minimum of one (1) year nursing experience required. Experience supporting individuals with IDD, home health, or long-term care preferred.
Technical Skills
- Proficiency in medication administration and clinical procedures
- Familiarity with electronic documentation systems
- Ability to use home-care medical equipment (feeding tubes, oxygen, suction equipment, etc.)
- Basic Microsoft Office and mobile application proficiency
Languages
Ability to communicate effectively in English, both verbally and in writing. Additional languages are considered an asset based on participant population needs.
Driver License
Valid driver’s license in good standing and reliable transportation required. Must meet OneWell Health Care motor vehicle eligibility requirements.
Travel Requirement
Local travel within assigned Pennsylvania service regions is required. Travel may include participant homes, community settings, and occasional attendance at meetings or trainings.
