Job Summary:
The RN Care Manager's primary responsibility is to provide care coordination for a set group of patients, adult or pediatric, with complex illness in the primary care setting, under the PCMH model. The RN Care Manager functions in an expanded, autonomous role, collaborating with all health team members to provide quality, efficient, and cost-effective healthcare. The RN Care Manager assesses the patient and family, implements a plan, coordinates care, monitors, and evaluates goals with frequent follow-up in accordance with PCMH guidelines. Integrates evidence-based guidelines and protocols in the development of patient-centered care plans and is involved in quality committees and initiatives.
Minimum Qualifications:
NYS licensed RN (or eligible), Bachelor's degree in Nursing or a healthcare-related field, and a minimum of three (3) years acute care, transitional care, or ambulatory experience OR an Associate's degree in Nursing and five (5) years of acute care, transitional care, or ambulatory experience required. CPR-BLS certification.
Preferred Qualifications:
Bachelor's degree in Nursing or healthcare-related field and prior care management experience preferred. Outpatient/ambulatory experience preferred.
