PRIMARY ROLE OBJECTIVE:
Creatively engage patients in preventive health education and health promotion activities to improve patient health outcomes. Interacts with patients with an understanding of social determinants of health and using a health equity framework.
MAIN DUTIES & RESPONSIBILITIES:
- Provides health education interventions such as: one-on-one interactions in a clinic setting, group classes, and community-based education and outreach. Some classes may be performed via video conferencing.
- Provides patient education as a clinical team member following co-visit procedures including documentation in the Care Plan section of the Electronic Health Record.
- Works with the Population Health Team to improve quality outcomes for patients of FHC by implementing clinical screening, education, and care management activities within scope and coordinate escalation of care based on patient complexity or need.
- Facilitate classes according to FHC guidelines.
- Initiates and maintains professional contact with community groups and organizations in order to increase accessibility and availability of resources for patients and to establish partnerships to strengthen health education programming.
- Assist marketing and QIP in developing health education materials in print and audio/visual formats targeting individuals with low health literacy and other languages.
- Evaluate, recommend, and disseminate culturally appropriate health education information and materials to a diverse audience.
- Maintains records of classes and attendance.
- Available to consult with providers and staff about health education needs.
- Participate in community needs assessments.
- Competent in basic health services such as first aid, blood pressure, diabetic foot checks, and height and weight measurements.
- Trained and competent in health-related screening tools.
KNOWLEDGE, SKILLS & ABILITIES:
- Requires more advanced organizational skills in order to organize projects, monitor deadlines, and maintain compliance with internal and external regulations and requirements.
- Job duties require the ability to work independently and as a part of both administrative and clinical teams.
- Job responsibilities require individual development of priorities for effective performance of duties, including re-prioritization in response to changes in circumstances.
- Work requires consideration of the way the work impacts other employees outside the department or functional area.
- Employees are expected to devise effective solutions to situations encountered based on the general goals and objectives of the function.
- Duties require the compilation, interpretation and reporting of information.
EDUCATION & EXPERIENCE:
Demonstrates competence in health-related topics with a combination of a Bachelor’s degree or higher in a health-related field. May substitute or show competence with certificates from accredited health education programs and/or pertinent work experience. Bilingual required English/Spanish.
ELIGIBILITY QUALIFICATIONS:
Holds a Community Health Worker certificate from WA DOH (may obtain within 90 days of hire).

ESSENTIAL FUNCTIONS:
Work is performed in an office/clinic setting. Work may occasionally require travel to other locations within FHC as well as throughout the county for community partner meetings and or trainings/conferences. FQHCs are a hybrid between a state and federal entity, and thus subject to government rules and criteria.
