Centurion is proud to be the provider of comprehensive healthcare services to the Indiana Department of Correction. We are currently seeking a full- time Associate Regional Director of Transitional Healthcare to join our team at the Indiana Regional Office in Indianapolis, Indiana.
The Associate Regional Director of Transitional Healthcare works under the direction of the Regional Director of Transitional Healthcare assisting in developing and implementing a transitional healthcare planning program for releasing incarcerated individuals statewide contract. Coordinates with IDOC, and other state or local agencies, to ensure that necessary pre-discharge planning and preparation is accomplished for individuals leaving the prison environment entering the community. Works closely with IDOC staff, Centurion transitional healthcare facilitators and liaisons, providers, and other staff assuring transitional care for releasing persons.
- Bachelor’s degree from an accredited program of Human Services, Social Services, or Criminal Justice program. Master’s Degree preferred.
- Minimum of one-year transitional/re-entry experience
- Must maintain current tuberculosis screening documentation and active CPR/AED certification
- Must be able to pass background investigation and obtain agency security clearance where applicable
- Must submit to and pass drug testing per agency requirements where applicable
- (2) years of related correctional experience or correctional healthcare experience, with a state correctional system preferred
- Must reside in Indiana
