Job Description
JOB SUMMARY
The Medical Social Worker LCSW at North Mississippi Health Services plays a pivotal role by coordinating assessment of psychosocial factors, providing direct case work, evaluating patient/family response to treatment, and communicating with patients, staff, and physicians. The Medical Social Worker LCSW is responsible for documenting patient care, providing mental health services including counseling and therapeutic interventions at the individual and group level, and developing treatment plans. They also are responsible for managing patient care fund budget and depending on care setting, can bill for therapeutic services. Extensive interaction with patients, families, hospital employees, physicians, and community resources necessitates strong human relations skills.
JOB FUNCTIONS
Supervisory:
- LCSWs can serve as supervisors/preceptors for LCSW candidates, LMSW and LSW where needed in health care settings.
- Occasionally assesses the Social Worker’s (BSW) treatment plans and quality of care.
Coordination:
- Coordinates clinical assessments of psychosocial factors, mental and emotional health and stability, direct case work, and community networking.
Evaluation:
- Evaluates psychosocial issues and patient/family response to treatment.
- Evaluates support systems and the strengths and weaknesses of the patient/caregiver environment.
Communication:
- Communicates goal progression and plan of care to patients, staff and physicians.
- Communicates agency mission and services to potential referral sources.
Records:
- Documents all patient care activities in a timely manner.
- Bills and collects for services rendered in certain care settings where they work as independent provider.
QUALIFICATIONS
Education
- Master's Degree In Social Work or Social Work Education. Required
Licenses and Certifications
- LCSW - License Clinical Social Worker. Currently be a "Licensed Certified Social Worker" in the State based on facility location. Upon Hire. Required
- LMSW - Licensed Medical Social Worker - State Licensure. current state license based on location of facility. Upon Hire. Required
Work Experience
- 1-3 years. Minimum of one year experience in a health care setting. Required
Knowledge Skills and Abilities
- Must be knowledgeable of basic medical terminology and major medical diagnoses.
- Must be knowledgeable of the principles of growth and development over the life span.
- Must have the ability to identify the appropriate needs and care of the patients served in Home Health and Hospice (neonates, pediatrics, adolescents, adults, and geriatrics).
- Must work independently with minimal guidance and supervision.
- Provides direct case work services.
- Provides mental health services for the prevention, diagnosis, and treatment of behavioral and emotional disorders for patients.
- Assesses the psychosocial factors.
- Documents all visits and counseling.
- Assists in identifying department goals.
- Performs work assignment according to rules and regulations.
- Attends in-service education programs, staff conferences, community resource meetings, and workshops.
- Assumes additional duties as needed.
- Oversees Social Worker (BSW) one time a month and conducts performance reviews.
- Manages patient care fund budget, expenditures and record keeping.
- Must work cooperatively and collaboratively with clinical/medical staff, office manager, all hospital and community agency personnel to enhance social services to patients/families, hospital and community.
- Time management is essential to ensure the patient’s needs are met in a timely manner.
- Must possess assessment skills in order to properly identify patients’ needs and act accordingly.
- Works with a wide variety of patients and home situations in order to assure quality patient care.
- Must develop good working relationships with patients and their families.
- Must contribute to a positive image of the Home Health and Hospice Department and North Mississippi Medical Center.
- Must be an advocate for the patients and the Agency and work accordingly with community, state and federal resources.
- Extensive interaction with patients, family members, hospital/clinical employees, physicians, community resources, office manager and Administration.
