INTRODUCTION
This role focuses on supporting hospice patients and their families through serious illness, end-of-life care, and bereavement. In addition, this role performs specialized hospice social work duties, including psychosocial assessment, counseling, goals-of-care support, advance care planning, and coordination with community resources as a member of the interdisciplinary hospice team.
Responsibilities also include identifying and addressing psychosocial, emotional, caregiver, financial, cultural, and practical needs related to terminal illness and decline; facilitating coping and adjustment; providing grief and bereavement support; assisting with care transitions and discharge planning; and advocating for patient autonomy, dignity, comfort, and access to appropriate home and community-based services.
This is a professional individual contributor role that may guide lower-level staff or support program processes. The majority of time is spent delivering and coordinating specialized hospice social work services using advanced clinical knowledge and sound judgment. This experienced-level role works independently with general supervision, manages sensitive situations that may be difficult but typically not highly complex, and collaborates closely with patients, families, caregivers, and the interdisciplinary team.
Responsibilities also include seeing patients in a variety of settings including home, hospitals, skilled nursing and assisted living facilities and our hospice inpatient unit, High Point House in Haverhill, MA.
LOCATION
Location: Southern New Hampshire Territories, including Merrimack, Pelham, Salem and Nashua
HOURS
Hours: Full-Time 8-4:30
MINIMUM QUALIFICATIONS
- Master’s degree in Social Work from an accredited institution.
- Licensed Clinical Social Worker (LCSW). Licensure in NH may be required depending on assigned territory and operational need.
- One (1) year of related experience.
